Abstract Background: Enamel decalcification in the form of white spot lesions (WSLs) is a common negative sequel of orthodontic treatment in the absence of proper oral hygiene. Although there is no golden standard for WSLs treatment, recently a new technology – CUROLOX - was introduced to biomimitically repair the decalcified WSLs via the selfassembling peptides. Aim of the study: To evaluate the efficacy of a self-assembling peptide in treatment of WSLs. Materials and methods: This study design was an in vitro prospective study that included 40 teeth extracted for orthodontic purposes. Artificial WSLs were created in the enamel and were randomly allocated into two groups. group 1 treated with self-assembling peptide while group 2 remained untreated. Subsequently both groups were kept in remineralizing solution at 37 C for 90 days. Color change and degree of mineralization were compared before and after the treatment. Results: The data was collected and statistical analysis were done using Statistical Package for Social Sciences ( SPSS ). No significant difference between groups was found after formation of WSLs ΔE (T0 - T1). Meanwhile significant difference was present between the two groups when comparing the color change after 90 days (T2-T1). ΔE values were 8.2 ± 4.81 and 2.13 ± 1.21 for the test and control groups respectively. Despite the final outcome showed marked improvement when compared to the baseline readings (T2-T0) 4.94 ± 3.66, the final color of the lesions was still not matching the baseline color. For the DIAGNOdent readings, in the test group the mean fluorescence value at T1 was 15 ± 3.81 while at T2 all specimens showed significant decrease in fluorescence values towards the value of sound enamel with a mean of 9.9 ± 1.94. For the control group, the mean value at T1 was 14.95 ± 2.16 while at T2 it was 13.65 ± 2.03 indicating minimal changes. Conclusion: Self-assembling peptide P11-4 ” REGENAMEL® ” shows promising efficacy to improve the color of WSLs , and promote remineralization of decalcified enamel surface.
