Abstract

Diffusion-weighted imaging is sensitive to changes in the micro
diffusion of water into both intracellular and extracellular spaces and
its use may improve MR characterization of gynecological pelvic
lesions.
Our study was conducted on sixty three gynecological pelvic
masses; The patients with benign tumors (No.=39) (62%), while those
with malignant tumors (No.=24) (38%) . after taking history we do
ultrasound examination to confirm the finding then we took consent
from the patient to do the examination after that we make sure that the
patient has no allergy to contrast and renal function is normal .
By performing DWI using different b-values, quantitative
analysis, namely, the calculation of apparent diffusion coefficient
(ADC) is possible and can be displayed as a parametric map (ADC
map). Restricted water diffusion demonstrates high signal intensity on
DWI and lower ADC values on ADC map.
After doing MRI with DWI we collect our data and presented
an individual analysis of the diagnostic performance of DWI during
the routine MR examination.
DWI had shown 95% (83%:100%) sensitivity in its individual
performance; yet a low specificity which was 54% (37%:70%). Such
low specificity value was attributed to the presence of 16 benign
masses that have mimicked malignancy on DWI .This means that
DWI is good sensitive but not good specific test, its relatively good
negative but bad positive test with low statistical significance,
meaning that the solo performance of DWI is not an applicable way
to discriminate benign from malignant gynecological masses
Conclusion
