Abstract

Breast cancer is impacting 2.1 million women each year, and causes greatest number of cancer related deaths among women. In 2018, it is estimated that 627,000 women died from breast cancer that is approximately 15% of all cancer deaths among women Worldwide (World Health Organization, 2020). In Egypt, cancer breast represents 18.9% of total cancer cases (32.04% in women and 2.2% in men) with an age-adjusted rate of 49.6 per 100 000 population. Cancer incidence and mortality display strong geographic patterns worldwide and in Egypt (Gewaifel, Bahnasy, Kharboush & Elsharkawy, 2019).
The psychological effects of breast cancer diagnosis and physical consequences associated with its treatment reduce the patients’ quality of life. The diagnosis and treatment; impaired sleep and activity patterns, physical symptoms and impaired cognitive function, dysfunctions in social and individual tasks threaten the quality of life. (Pitman, Suleman, Hyde & Hodgkiss, 2018)
Aim of the study:
The aim of this study is
• Assessquality of life among breast cancer patients during adjuvant chemotherapy.
Research questions:
• What is the quality of life among breast cancer patients during adjuvant chemotherapy
Subjects and Methods:
Design-:
A descriptive exploratory researchdesign was utilized to conduct this study.
Setting:
The present study was conducted at Damanhur Oncology Center in the outpatient chemotherapy clinic; this clinic located in the second floor and consists of 2 rooms, each room contains 10 beds at Damanhur Oncology Center.
Subjects:
A convenient sample of female patients (110) diagnosed with breast cancer was included in this study from the above mentioned setting.
Inclusion criteria:
• Females 18 years and above
• Married
• Womem who received chemotherapy
Tools for data collection:
1- A structured interviewing questionnaire:
It was developed by the researcher in simple Arabic language based on the extensive review of relevant and recent literatures to assess. Itwill include the following:
Part (1): Socio demographic characteristics of the studied patients such as: age, educational level, place of residence, occupation and income
Part (2): clinical data of the studied patients such as: receiving of radiotherapy, duration since starting of chemotherapy and family history.
2- Quality of Life Instrument - Breast Cancer Patient:
The standard tool was adopted by Ferrell, Dow and Grant (1995) to represent the four domains of quality of life including physical well being, psychological well being, social well being and spiritual wellbeing, it was translated into Arabic languageby the researcher and assessed by five experts; three experts from Psychiatric-Mental Health Nursing in and two experts from Medical-Surgical Nursing in Faculty of Nursing/Ain Shams University and no modification was done after jury.
Results:
The results of the study showed that:
- The study revealed that more than one third (34.5% ) of women with breast cancer were in age group 50 years, and less than three fifths (58.2%) were Illiterate, more than three fifth (69.1% ) of them were Housewife, more than half (53.6%) of them their family income was not enough and more than three fifths (68.2%) live in rural area.
- Slightly more than three fifths (65.5%) of them had no family history, and more than half (57.3%) of them did not receive radiotherapy. As regard Chemotherapy duration half of the study sample (50%) received chemotherapy for five years.
- The result reveals that more than three quarters (77.3%) of patients with breast cancer under study had low total quality of life at physical domain, while less than one quarter (22.7%) of them had high quality of life.
- The highest percentage (95.5%) of patients with breast cancer under study had low total quality of life at psychological domain, while only (4.5%) had high quality of life at psychological domain.
- The highest percentage (89.1 %) of patient with breast cancer under study had low total quality of life in social concerns, while only (10.9 %) had high quality of life in social concerns.
- And less than two thirds (65.5%) of patient with breast cancer under study had high quality of life in spiritual well-being, while more than one third (34.5 %) of them had low total quality of life in spiritual well being.
- Also, the highest percentage (90.9 %) of patients with breast cancer under study had low total quality of life, while only (9.1 %) had high quality of life.
- There were highly statistically significant relations between socio-demographic characteristics of patient with breast cancer under study and their physical wellbeing.
- There were highly statistically significant relations between socio-demographic characteristics of patient with breast cancer under study and psychological wellbeing.
- There was a statistically significant relation between Job condition of patient with breast cancer under study and social wellbeing, and there were highly statistically significant relations between age, level of education, family income, and place of residence of them and social wellbeing.
- There was a statistically significant relation between place of residence of patient with breast cancer under study and spiritual wellbeing in, and there were highly statistically significant relations between age, level of education, job, and family income of them and spiritual wellbeing.
- There were highly statistically significant relations between socio-demographic characteristics of patient with breast cancer under study and total QOL.
- There were highly statistically significant relations between clinical data of patient with breast cancer under study and physical wellbeing.
- There were highly statistically significant relations between clinical data of patient with breast cancer under study and psychological wellbeing.
- There were highly statistically significant relations between clinical data of patient with breast cancer under study and social wellbeing.
- there were highly statistically significant relations between clinical data of patient with breast cancer under study and spiritual wellbeing
- There were highly statistically significant relations between clinical data of patient with breast cancer under study and total QOL.
- There were positive statistically significant correlations between total psychological domains and total physical domains also total QOL and total spiritual domains. Additionally, there were highly positive statistically significant correlations between total social domains and total physical domains, also between total QOL and total physical, total psychological and total social domains.
