Abstract Behcet’s disease BD is a relapsing multisystem inflammatory disease. Recurrent oral ulcers are the most common clinical manifestations in BD, present in almost all patients, followed in descending order by genital ulcers, erythema nodosum and papulopustular lesions followed by arthritis, uveitis, thrombophlebitis, gastrointestinal and central nervous system involvement. Most BD patients initially manifest with recurrent oral ulcers with a frequency of 97–100% while genital ulcers vary from 62 to 100% of patients. Pseudo folliculitis, papulopustular eruption, and erythematosus are the most common skin manifestations of BD. In BD, cardiovascular involvement occurs in 7–46% in the form of pericarditis, myocarditis, endocarditis, valve diseases, endomyocardial fibrosis, and intracardiac thrombosis. Pulmonary manifestations are reported in 0.3–18% of BD cases and the patterns are quite heterogeneous and include pleural effusion, pulmonary arteritis or venulitis, emphysema, pneumonia, chronic bronchitis, and fibrosis. Pulmonary artery aneurysm is characteristic of BD and carries a poor prognostic sign. The study primary aimed to explore patient reported outcome (PRO) in the form of signs and symptom, quality of life, and disease impact on daily activities. This was an observational, cross-sectional, study conducted on total of 100 patients during the period from September 2019 to Abril 2021. Patients fulfilling the International Study group (ISG) criteria for BD. All patients were selected according to the inclusion and exclusion criteria: Inclusion criteria: Aged more than 18 years old, both gender and patients fulfilling the International Study group (ISG) criteria for BD. Summary 64 These criteria require the presence of oral ulceration plus any two of genital ulceration, typical defined eye lesions, typical defined skin lesions, or a positive pathergy test. Exclusion criteria: severe physical disability, patients with hypertension, diabetes mellitus, or ischemic heart diseases and patients who refused to participate in the study. These patients were subjected to the following: Personal history taking, disease history (age at onset, disease duration, family history and signs and Symptoms of BD), Laboratory Investigations including (Complete blood picture (CBC) with differentials, inflammatory markers (CRP and ESR), liver function tests (Alanine aminotransferase (ALT) and aspartate aminotransferase (AST)), renal function tests (serum creatinine levels) and imaging including findings of plain X-ray and CT scan. Results of the current study can be summarized as follows: There was male predominance among our patients (73%). The mean age at onset and Disease duration of the studied patients were (27.31±3.85 and 10.55±18.46) years respectively. 22.7%, of patients hand positive family history. Most of the studied patients (36.8%) had bored faces over the four weeks. Headache, mouth ulceration, genital ulceration, erythema, skin pustules, joints of (arthralgia and arthritis), nausea/vomiting/abdominal pain and diarrhea+ altered/frank blood per rectum were found with percentage of (63.8%, 72.4%, 49.7%, 27.6%, 41.1%, 63.8%, 9.2%, 22.7%, 36.8%) respectively. Most of the studied patients had redness, pain, blurred or reduced vision in right eye with percentage of (77.3%, 58.9%, 77.3%) Summary 65 respectively. Also, in left eye with percentage of (63.8%, 49.7%, 77.3%) respectively. Difficulty with speech, blurring of/double vision, weakness or loss of feeling of face and arm were present in (13.5%, 90.8%, 9.2% and 27%), respectively. Chest pain, breathlessness, pain/swelling/discoloration of (face, arm and leg), new active major vessel inflammation and bred faces over the last 4 weeks were found in (31.9%, 13.5%, 27.0%, 22.7%, 36.8%, 14.1%, 36.8%) of patients, respectively. The mean hours missed from work because of problems associates and hours on work were (32.79±33.76, 45.25±68.08, 17.88±20.57) respectively. About (28.8%) of the patients had very mild affecting problem on the work and the ability to do regular daily. There were significant correlations between SF 36 with age at onset and headache. |
