Abstract

Purpose: To compare the outcome of anterior lamellar recession (ALR) and tarsotomy surgery for the treatment of lower lid trachomatous trichiasis as regards trichiasis recurrence and cosmetic outcome.
Methods: This comparative prospective study included 49 eyes of 40 patients with lower eyelid trachomatous trichiasis; divided to two groups; group A underwent Anterior Lamellar Recession procedure ALR (20 patients with 24 eyes), and group B underwent Tarsotomy procedure (20 patients with 25 eyes). Outcome measures included trichiasis recurrence and cosmetic outcome.
Results: follow-up time of 6 months (range, 6.0 – 8.0 months).The patients achieved good ultimate cosmetic outcome in both groups. The intergroup difference was not statistically significant (P > 0.05(.The mean trichiasis recurrence rate in ALR group is 4.2% and in Tarsotomy group is 20% at the end of follow up period.
Conclusion: ALR procedure is proved in to be competent and more efficient in handling misdirected and metaplastic lashes. The cosmetic results of all patients were good or acceptable compared to tarsotomy group.
