Abstract

TB remains a growing threat to public health inspite of developed treatment at last years.
The aim of the current prospective study is to study the frequency of bronchiectasis in TB patients in duration of 1 year from 1/1/2020 to 31/12/2020 and to evaluate prevalence and burden of bronchiectasis in TB patients and in Assiut chest hospital.
Patients were evaluated by demographic data, symptoms and radiological. Final diagnosis was dependent on bacteriological samples (sputum, pleural effusion and biopsies)
All samples were collected for AFB, gene expert and TB culture.
The study included 198 patients with suspicion of TB, 98 were definite diagnosed as TB patients, 9 were extra-pulmonary TB, 89 were pulmonary TB, 3 of 89 did not follow up, 1 patient died at the beginning of the study, and we completed the study with 85 patients.
In the current study 19 of 85 cases were diagnosed as bronchiectasis either before or after diagnosis of TB.
Regarding to demographic data, bronchiectasis is more common in males than females, married, urban, low SES and smokers.
Clinically, the most common symptoms of bronchiectasis are hemoptysis and expectoration.
Other symptoms may be found like dyspnea and chest pain and psychic trauma and fever and weight loss.
Regarding to radiological findings, cylindrical is the most common type of bronchiectasis.
And the most common findings in CT are lack of tapering of bronchus, wall thickening with mucous plug and tree in bud pattern.
Regarding to burden of the disease, There is statistically significant difference between the studied groups regarding frequency of hospitalization, length of hospital stay, frequency of outpatient visits, days of antibiotic use over year and need for oxygen therapy. All were significantly higher in patients with bronchiectasis.
There are 2 scores for assessment of bronchiectasis, BSI and FACED scores. BSI is better than FACED in evaluation frequency of outpatient visits. FACED is better than BSI in evaluation of frequency of hospital admission and length of stay in a hospital and expecting mortality.
