Abstract

Background: Knee osteoarthritis (OA) is a serious chronic disease and disability worldly and this imposes high costs to the health systems. Osteoarthritis pathology starts by the destruction of articular cartilage and bones engage under the cartilage resulting in inflammation of surrounding tissue. knee OA is characterized by pain and movements limitation that affect quality of life, social and economic activities. This study aims to compare the outcomes of hyaluronic acid (HA) versus platelet-rich plasma (PRP) in the managment of knee osteoarthritis.
Patients and Methods: This is a systematic review of the literature with a meta-analysis study on randomized clinical trial studies between 2010 till 2019. WOMAC score is the outcome in follow up of 0,1,3,6,12 months.
Results: Hyaluronic acid has better performance than plasma rich protein immediately after injection (MD of 3.621, 95% Cl 0.072-7.170; P<0.001, I²= 93.25%). After one month, the PRP has mild better performance than HA (MD of -0.804, 95%Cl -1.687-0.080; P=0.002, I²= 79.36%). The performance of plasma rich protein continued to be better than the hyaluronic acid after 3 (MD of -1.598, 95%Cl -3.513 to 0.317; P<0.001, I²= 96.23%), this preference continued after 6 and 12 months of injection.
Conclusion: Intra-articular injection of PRP more effective than HA in knee osteoarthritis management. This preference is in pain relief, physical function and stiffness progression as well as continue till one year after injection.
