Abstract

Behçet’s syndrome (BS) is a systemic inflammatory vasculitis of unknown aetiology, affecting vessels of different types, sizes and locations. BS is mainly characterized by recurrent episodes of acute inflammation, including mucocutaneous manifestations (oral aphthous ulcers, genital ulcers and skin lesions) and gastrointestinal, musculoskeletal, neurological, ophthalmic and vascular involvement. However, there is considerable variation in its clinical presentation between and within individuals.
The burden of BS is usually confined to the initial years of its course, with most symptoms abating with the passage of time. Mucocutaneous manifestations are the most common symptoms in adults with BS while eye, vascular and neurological involvement are the most serious. The complex patterns of symptoms in adults with BS can cause various degrees of activity limitation and restriction in everyday life.
Although previous studies have shown the impact of BS on quality of life, most of them focused on the influence of one particular symptom, such as cutaneous manifestations, intestinal problems, ocular involvement, arthritis, oral ulcers and major depression. The remaining studies looked at the relationship between disease activity and quality of life or compared the quality of life of BS patients with other groups. No studies have explored the differential or cumulative impact of multiple symptoms on quality of life.
The aim of this study was to evaluate the quality of life (QoL) in patients with Behçet’s disease in three center experience especially Menoufia university hospital.
This study was a cross sectional study, the study was carried out on 70 Patients with Behçet’s disease selected from Rheumatology and immunology unit from Menoufia university hospital Benha teaching hospital, Mansoura university hospital and Hod marsoud hospital from January 2019 till December 2020 on patients that attended diagnosed by trained well qualified dermatologists & andrologists, all participants were assessed by the SF-36 QoL Scale was used to evaluate the QoL in patients diagnosed with Behçet’s disease.
The main results of the study revealed that:
There were 54(77.1%) male and 16(22.9%) female. Nineteen patient (27.1%) aged below 20, 16(22.9%) aged between 20-30, 28(40%) aged between 30-40 and 7(10%) aged above 40. The mean age was 26.88 ± 10.31, the mean weight was 66.26 ± 13.61, the mean height was 164.0 ± 9.64, the mean BMI was 24.46 ± 3.96. As for smoking there were 53(75.7%) with negative smoking and 17(24.3%) with heavy smoking.
There were 11(15.7%) with HTN, 3(4.3%) with DM.
There were 19 (27.1%) with mild oral ulcer, 12 (17.1%) with moderate oral ulcer and 39 (55.7%) with severe oral ulcer.
regardly for genital ulcer there were 9 (12.9%) with improved Genital ulcer, 18 (25.7%) with mild Genital ulcer, 10 (14.3%) with moderate Genital ulcer and 11 (15.7%) with severe Genital ulcer. As for Eye there were 28(40%) with Ant uvitis, 39(55.7%) with Post uvitis and 8(11.4%) with R.A.T.

regardly for CNS Affection there were 63(90%) with negative CNS affection and 7 (10%) with dural sinus thrombosis. All patients were negative patheragy.
regardly for Arthritis, there was 60 (85.7%) with negative Arthritis and 10 (14.3%) with Unilateral ankle arthritis.
Regardly for stroke there were 62 (88.6%) with negative stroke and 8 (11.4%) with dural Sinus thrombosis. There were 25(35.7%) had DVT.
The mean systolic BP was 124.6 ± 11.68 with (range 110-155), the mean Diastolic BP was 72.29 ± 9.95 with (range 60-95).
The mean ESR 51.29 ± 12.82 with (range 30.0 – 70.0), the mean CRP 28.77 ± 13.24 with (range 8.0 – 48.0), the mean FBS 80.36 ± 6.04 with (range 70.0 – 90.0), the mean 2hr pp 123.9 ± 7.36 with (range 110.0-135.0).
According to Emotional problems interfered with patients normal social activities with family, friends, neighbors or groups there were 25(35.7%) with grade 1, 21(30%) with grade 2, 24(34.3%) with grade 3.
According to bodily pain patients had during the past 4 weeks there were 23(32.9%) with grade 1, 24(34.3%) with grade 2, 23(32.9%) with grade 3.
there is no significant difference between total QOL and age, no significant difference between total QOL and Weight, no significant difference between total QOL and Height, no significant difference between total QOL and BMI, no significant difference between total QOL and BP(systolic nor diastolic), no significant difference between total QOL and ESR, no significant difference between total QOL and CRP, no significant difference between total QOL and FBS, no significant difference between total QOL and Hr2 pp.
Based on our findings, we recommend for further studies on lager sample size and on large geographical scale to emphasize our conclusion.
