Abstract

Introduction: Cooling dialysate first came into practice three decades ago after it was noted the symptoms of patients suffering from IHD on dialysis. Many benefits of cooled dialysate temperatures have come to light. Multiple studies have demonstrated improved hemodynamic tolerance on dialysis specifically in patients prone to IDH without any adverse effect on dialysis adequacy. It is an inexpensive intervention that also reduces the frequency of nursing involvement to address IDH in patients on chronic haemodialysis. While the cooler temperatures may cause discomfort in some patients, a temperature of 0.5 °C below an individual’s CBT is better tolerated than an arbitrary temperature of 35 °C. The Aim of this study: The present study aimed to study the effect of cool dialysate on echocardiographic parameters in patients on maintenance hemodialysis. Materials and methods: The present study was carried out on sixty-seven patients treated by chronic hemodialysis for more than six months. The enrolled patients were divided into 2 groups: The first group (intervention group): cooling dialysate (n=37).The second group (control group ):non cooling dialysate (n=30).Medical history taking,basic laboratory investigations and echocardiography were carried out. Results: At the end of the study, there was a statistically significant decrease in serum PRO BNP and hemoglobin and significant increase in serum phosphorus and LV end diastolic volume in patients dialyzed with cool dialysate. On the other hand patients dialyzed without cold dialysis had statistically significant increase in serum PRO BNP and significant decrease in PTH, ferritin, hemoglobin and LV End systolic volume. While stroke volume and ejection fraction increased in both groups. There was significant decrease in SBP at the end of the study in patients dialyzed without cool dialysate. There was a statistically significant negative correlation between BNP and LV mass and a statistically significant positive correlation between BNP and Septal wall thickness at end diastole at the start of the study, while at the end of the study there was a statistically significant negative correlation between BNP and Ejection fraction and Fractional shortening. On multivariate analysis LVM could be significantly predicted by age, baseline Left ventricular mass and baseline PRO_BNP level,while KT/V was the only predictor of LV End diastolic volume.
