Abstract

chronic urticaria (CU) is a common skin disorder characterized by intermittent recurrent erythematous-edematous lesions (wheals and flare reactions) with or without associated angioedema for at least six weeks (Zuberbier et al., 2014). The association between CSU and MetS as whole or individual components might support the existence of shared factors and/or mediators. Systemic inflammation is a phenomenon common for MetS and CSU acting as a possible link between the two conditions (Vena and Cassano, 2017). The aim of this work is to estimate the levels of serum leptin, adiponectin and other metabolic syndrome related parameters and their correlation with occurrence and severity of CSU which might help in better understanding of the pathogenesis of chronic spontaneous urticaria. The present study comprised 44 patients diagnosed as chronic spontaneous urticaria and 46 normal volunteers act as control (age and sex matched). All participants were subjected to careful history taking, systematic general examination, full dermatological examination and assessment of urticarial activity by urticaria activity score (UAS). Metabolic syndrome is diagnosed by the presence of 3 or more of the National Cholesterol Education Program’s Adult Treatment Panel III (NCEP-ATP III) criteria. Serum levels of leptin and adiponectin were determined by an immunometric sandwich enzyme-linked immunosorbent assay (ELISA) using commercially available kits named human leptin (LEP) and adiponectin (ADP) ELISA kit. 1. CSU most frequently occurred in middle age (39.75±7.74) years. 2. Median duration of disease/months was 13.5 months. 3. Majority of cases have mild activity CSU (54.5%) and moderate or severe activity CSU represent (15.9%) of CSU cases. 4. Metabolic syndrome was diagnosed in about (34.1%) of patients with CSU. 5. Patients with metabolic syndrome have higher mean age, median UAS range, median duration of urticarial symptoms, BMI, waist circumference, percentage of hypertension, FBG, percentage of DM and TGS with lower HDL than those without metabolic syndrome. 6. Median serum leptin level in CSU patients was (65.25 ng/ml) which was significantly higher than the median level in the control group (52.75ng/ml). 7. Median serum adiponectin level in CSU patients was (5.2 mg/L) which was lower than the median level in the control group (5.45 mg/L). 8. Median serum leptin level in CSU patients with metabolic syndrome was (122.2 ng/ml) which was significantly higher than the median level in CSU patients without metabolic syndrome (53.8 ng/ml). 9. Median serum adiponectin level in CSU patients with metabolic syndrome was (5 mg/L) which was significantly lower than the median level in CSU patients without metabolic syndrome (5.5 mg/L). 10. There was positive (not significant) correlation between UAS7 and serum leptin. 11. There was negative (not significant) correlation between UAS7 and serum adiponectin.
