Abstract

The treatment of patients with severely resorbed mandible using conventional complete dentures is mainly associated with several problems.
Implant mandibular assisted over denture is one of the treatment options that increase the patient satisfaction especially masticatory efficiency.
The most common problem in implant dentistry is the presence of insufficient bone volume for insertion the dental implant of conventional size.
The use of narrow-diameter implants instead of ridge augmentation can simplify and facilitate the treatment of such cases.
A wide variety of methods have been used to determine masticatory functions.
This study aimed to evaluate the masticatory efficacy of both masseter and anterior temporalis muscles during chewing and clenching when using the three different treatment protocols.
In this study Electromyograph (EMG) was used to evaluate and compare the activity of masticatory muscles in different situations.
Eight completely edentulous patients were selected for this study. Complete maxillary and mandibular dentures were fabricated for each studied patient. After two months, four narrow diameter implants were placed in the mandibular arch. After healing period for optimum osseointegration (three months), two implants mandibular assisted over denture was constructed, then after two weeks four implant mandibular assisted over denture was constructed.
Electromyograph activityof masticatory muscles was evaluated for each studied patient during the three different situations i.e. CD, two IOD and four IOD.
Data were fed to the computer and analyzed,the following were resulted:
During clenching and chewing, the mean value of EMG activity of masseter and anterior temporalis muscles of IODs was significantly higher than in conventional CDs. In addition, the mean value of EMG activity of four IOD was significantly higher than that of two IOD.
Also, the clinical and radiographic parameters around narrow implants were evaluated periodically after three and six months of implant function and showing changes within normal range which were compared with that of standard diameter implant.
This study was concluded that:
• Using narrow implants improve the masticatory efficiency of the patient.
• Implant number have appositive impact on the two masticatory muscles activity.
• Four narrow implants mandibular overdenture is a successful treatment and can be used as alternative treatment option for patients unable to receive conventional implant.
