Abstract

Percutaneous endoscopic gastrostomy (PEG) is a medical procedure used for enteral feeding with the use of the natural digestion process of the gastrointestinal tract; PEG is performed using endoscopic guidance to insert a tube into the patient’s stomach through the abdominal wall, The PEG procedure is an alternative to open surgical gastrostomy insertion, and does not require a general anesthetic; mild sedation is typically used. Nursing team members are responsible for direct and continuous care related to PEG by assessing the tube and the skin around it and to avoid complications (Rahnemai. Et al., 2014)
Aim of the study:
This study aimed to assess nurse’s performance regarding care of patients post percutaneous endoscopic gastrostomy in critical care unit, it would be achieved though:
1. Assessing nurses level of knowledge regarding care of patients post percutaneous endoscopic gastrostomy in critical care unit
2. Assessing nurses level of practice regarding care of patients post percutaneous endoscopic gastrostomy in critical care unit
Research question:
1. What are nurses level of knowledge regarding care of patients post percutaneous endoscopic gastrostomy in critical care unit?
2. What are nurses level of practice regarding care of patients post percutaneous endoscopic gastrostomy in critical care unit?
Subjects and Methods
Research design:
In the current study a descriptive explaratory study design was used to achieve aim of this study.
Setting:
The current study was carried out at four different intensive care units of Ain Shams University Hospital. These units are:
First: Medical Intensive Care unit 1 in the 1st floor which consisted of (17 beds).
Second: Medical Intensive Care unit 2 in the 1st floor which consisted of (17 beds).
Third: Cardiac Care Unit in the 2nd floor which consisted of (19 beds).
Fourth: Surgical Intensive Care unit in the 2nd floor which consisted of (42beds).
Subjects:
All available nurses’ working at ICU and caring for patients post PEG tube in the previously mentioned settings (30 nurses) were agreed to participate in this study.
Tool of data collected
Two tools were constructed and tested by the investigator to collect data pertinent to this study these tools were:
Nurses self-administered questionnaire tool:
It compromised Arabic structured items related to different aspects of assessment knowledge regarding care of patients post percutaneous endoscopic gastrostomy in critical care units, it consisted of the following:
Part I: demographic data.
Part II: was concerned with assessment of the following:
First part: Anatomy and physiology of GIT system (Five questions).
Second part: General knowledge about PEG. (Six questions).
Third part: Indications of PEG. (Six questions).
Fourth part: Contraindications of PEG. (Five questions).
Fifth part: Complications of PEG. (Seven questions).
Sixth part: Nursing management post PEG placement. (Five questions).
Seventh part: Nursing management before administration of feeding or medications. (Five questions).
Eighth part: Nursing management during administration of feeding or medications. (Seven questions).
The scoring system:
The questions were in the form of multiple choice and true and false questions. One grade was given for each correct answer, and zero for the incorrect one. The total grades for every part were summed and percentage was calculated.
The knowledge level was categorized into satisfactory and unsatisfactory as follow:
• Satisfactory ≥ 85% ( ≥ 39 degrees).
• Unsatisfactory < 85 % ( < 39 degrees).
Nurses’ practice observational checklists
It comprised 7 parts covering the following:
• Practices related to preparation of PEG care (before the procedure) consists of 10 steps.
• Practices regarding care of PEG tube at golden hours consist of 7 steps.
• Practices regarding continuous care of PEG tube consist of 8 steps.
• Practices regarding continuous feeding via PEG tube consist of 21 steps.
• Practices regarding administration of a bolus feed via PEG tube consists of 11 steps.
• Practices regarding administrating medication via PEG Tube consist of 5 steps.
• Practice after the procedure consists of 6 steps.
The scoring system:
The total scores for every part and for all checklists was calculated and changed into percentage, every step was correctly done given one grade, and zero for the not done or done incorrectly. The total grades for every part were summed and percentage was calculated:
• ≥ 85% ( ≥ 58 degree) considered satisfactory level of practice
• < 85% ( < 58 degree) considered unsatisfactory level of practice.
