Abstract

Self-esteem is child’s passport to a lifetime of mental health and social happiness. It’s the foundation of a child’s well-being and the key to success as an adult. At all ages, how you feel about yourself affects how you act. Think about a time when you were feeling really good about yourself. You probably found it much easier to get along with others and feel good about them. Self-esteem is a concept used to describe a personality variable that captures the way a child generally feels about himself, or the way a Child may evaluate his abilities and attributes.
Also child with high self-esteem generally has a fondness or love for himself, whereas a child with low self-esteem may have negative feelings toward himself, The lowest levels of self-esteem were observed in children with chronic fatigue syndrome associated with chronic renal failure. Lower levels of self-esteem in children with a chronic illness were found in girls than in boys, in adolescents than in children . Psychosocial interventions for children with chronic illnesses should be offered for children with low self-esteem.
Self-efficacy is the degree to which a Child believes that he or she can attain a goal. It is also an important part of treatment for mental health disorders, as a high degree of self-efficacy correlates with a higher chance for treatment success. Self-efficacy, a widely used psychological concept, has been recognized as an essential prerequisite of effective self-care of chronic disease Such as chronic renal failure. Education researchers consistently note that a child with higher self-efficacy works harder, is more optimistic and less anxious, and perseveres more.
Aim of the study:
This study aimed to evaluate the effect of nursing intervention program on self-esteem and self-efficacy of children undergoing hemodialysis.
Hypothesis
The implementation of the nursing intervention program will have a positive effect on both of self-esteem and self-efficacy of children undergoing hemodialysis.
Subjects and methods:
The technical design includes research design, setting ,subjects and tools of data collection :
a. Research design:
A quasi-experimental research design (pre and posttest) was used for the current study.
b. Research setting:
This study will be carried out on children undergoing hemodialysis at the hemodialysis unit of menoufia University hospitals.
c. Research subjects
Sample:-
A convenient sample of all available number of chronic renal failure children who are undergoing hemodialysis. Their number is about (58 child), their age ranging from 6 to 12 years old.
Ethical consideration:
The ethical research considerations in this study include the following:
6. A written initial approval was obtained from the research ethical committee at the Faculty of Nursing, Ain Shams University.
7. Children oral consent was obtained from each participating child after explaining the nature and benefits of the study.
8. The researcher cleared the objectives and aim of the study to participating children.
9. The researcher maintained anonymity and confidentiality of participating children .
10. Participating children were allowed to choose to participate or not in the study, and given the right to withdraw at any time from the study without giving reasons.
Tools of data collection:
Tools for collecting data of the present study will include the following:
1- Interview questionnaire sheet will be developed by the researcher based on literature review will be consisted of:
a- Demographic data such as ( Age ,Sex ,Education ,etc…)
b- History of the disease.
2- Self esteem scale to assess self esteem of children undergoing hemodialysis. It will be quoted from (Rosenberg, 1965) with scoring system of scale.
3- Self efficacy scale to assess self efficacy of children undergoing hemodialysis. It will be quoted from (Lorig etal.,1996) with scoring system of scale.
Results:
The results of the study revealed the following:
1. More than three quarters of the sample (80%) have low self-esteem pre intervention program. which decreased to (12%) post intervention program and continue decreasing to (4%) during follow up.
2. More than three quarters of the sample (82%) have low self-efficacy pre intervention program. which decreased to (4%) post intervention program and increasing to (6%) during follow up.
3. There was statistically highly significant relation between self-esteem and self-efficacy pre and post intervention program.
4. There was statistically highly significant relation between father education, mother education, interaction between father and mother and standard of living with both of self-esteem and self-efficacy.
5. There was statistically highly significant relation between duration in hemodialysis, Fatigue during hemodialysis sessions and the numbers of hemodialysis sessions per week with both of self esteem and self efficacy.
Conclusion
The findings of the present study reached to the following conclusion:
*Children undergoing hemodialysis have low levels of both of self-esteem and self-efficacy pre intervention program .
*There is an increasing and enhancement of levels of both of self-esteem and self-efficacy of children undergoing hemodialysis post intervention program and in follow up.
*There is a strong positive correlation between self-esteem and self-efficacy among children undergoing hemodialysis pre and post intervention program.
Recommendations:
Based on the findings of the present study, the following recommendations are suggested:
1-Plan an educational program and training program for health care givers, parents and teachers about how to foster both of self esteem and self-efficacy in children undergoing hemodialysis.
2-Media attention to the phenomenon of low self-esteem and low self-efficacy in children undergoing hemodialysis and provide broadcast awareness programs help to solve the problems of this phenomenon.
3-Provide remedial programs for children undergoing hemodialysis aims to rid them from their low self-esteem and low self-efficacy and problems arising from it such as depression, anxiety and stress.
To further researchers:
Further studied should be conducted on self esteem and self efficacy in children undergoing hemodialysis and its relationship with depression and anxiety
