Abstract

The aim of this comparitive prospective study is to assess the best appropriate screw entry zone based on alpha angle in volar percutaneous headless compression screw fixation of acute scaphoid fracture. Another point of concern is the efficacy of SSA in detection of prognosis (healing and functional outcome).
30 patients with acute scaphoid fractures divided into 2 groups (10 in the control group and 20 in the study group), all are waist fractures Herbert type B1 and B2.
Depending on the orientation of fracture line we proposed an entry zone for fixation, unrestricted mobilization was allowed after 4 weeks, post operative radiographic and functional assessment was obtained and complictions were evaluated.
Clinical outcomes were evaluated according to DASH score, MMS score and VAS score.
In the control group union was achieved in 8 cases (80%) while union in 19 cases (95%) was achieved in the study group. One case in each group developed superficial infection which was treated by short course of antibiotics and improved.
One case in each group developed stiffness in wrist after cast removal, which improved by physiotherapy.
According to MMS , the control group was resulted in 3 cases (30%) were excellent, 5 cases (50%) were good, 1 case (10%) was fair and 1 case (10%) was poor. While the study group was resulted in 15 cases (75%) were excellent, 4 cases (20%) were good, 1 case (5%) was fair and no case was poor.
