Abstract

Instructional Guidelines for Patients with Pott’s Fracture to Improve Physical Activity and Functional Ability Pott’s fracture, also well-known as Pott’s syndrome and Dupuytren fracture, is an ancient term loosely applied to a variety of bimalleolar ankle fractures (Hunter, Peltier, and Lund, 2000). Fractures of the ankle joint are among the most frequent fractures encountered in any accident and emergency department (Tengberg and Ban, 2018). Pott’s fracture may lead to physical inactivity and subsequent mobility limitation and disability. Regular physical activity and healthy guideline has been associated with better outcomes in the post pott`s fracture period, medicare reports that 8.3 out of every 1000 Medicare recipients experience an ankle fracture each year (Barile et al, 2017). This study aims to: Determine the effect of instructional guidelines for patients with pott’s fracture to improve physical activity and functional ability. Research Hypotheses: The application of guidelines among pott`s fracture patients can lead to the following: H1- There will be improvement of patient knowledge pre and post application of guidelines. H2- There will be improvement of stability of ankle, ability to walk, to run, to work motion of ankle after application of guidelines and speed of recovery. Instructional Guidelines for Patients with Pott’s Fracture to Improve Physical Activity and Functional Ability 72 H3- improvement of physiologic measures of pain, radiographic results compared with pre application of guidelines. Design: Quasi-experimental design was utilized in this study. Setting: This study was conducted at orthopedic unit of Emergency hospital & Accident Hospital at Mansoura university. Subjects: Purposive study samples were 52 patients according to sample size. Accordingly, patient`s sample sizes of 52 patients were assigned before giving guidelines and after giving guidelines. Inclusion criteria:  Aged between 20–60 years old.  Both sexes.  Patient has pott’s fracture and able to communicate and to participate in physical exercise. Exclusion criteria:  Psychiatric disorders, quadriplegia, certain behavior or cognitive disorders (e.g., aggressive behavior, impulsivity).  Cancer patient especially (bone cancer). Instructional Guidelines for Patients with Pott’s Fracture to Improve Physical Activity and Functional Ability 73  Pathological fractures Tools used to collect the pertaining data are: 3 Tools Tool I: Assessment interview questionnaire sheet includes demographic and health relevant data Tool II: Health Knowledge Assessment Sheet Tool III: Scoring System of Baird and Jackson. The study finding revealed that: The highest proportions in demographics characteristics of study subject were to male 31 (59.6%) and their age ranged from 20 to 60 yrs. The median age of study patient`s was 39 years. More than half of the study (53.8%) work needs movement. Around three-quarters (76.9%) were able to read and write. Around three-fifths of the study (61.5%) were married with only a minority (11.5%) living alone. Improvement in knowledge of study sample post guidelines implementation VS pre guidelines implementation where the median total score (and percentage) was 25/37 (67.6%) post guidelines implementation vs. 9/37 (24.3%) pre guidelines implementation. There was statistically significant higher score of Baird and Jackson’s Scoring System with P value (<0.001) post implementation guidelines vs. pre implementation with median total score was (95.3%) good to excellent result post guidelines vs. (52.1%) poor results pre guidelines. The study finding concluded that: A statistically highly significant BJSS score and knowledge after guidelines, this indicates that the instructional guidelines is significantly effective in improving physical activity and functional ability among pott`s fracture patients. No statistically significant correlation between BJSS &knowledge and demographic data has any statistically significant with BJSS &knowledge. The study recommended that:  Handout with instructional guidelines should be distributed and be available for every Pott`s fracture patient admitted to orthopedic department, which are supported by the presence of a nutritionist and exercise trainer.  Further studies are needed in order to develop more accurate and realistic strategies for improving physical activity and functional ability for Pott`s fracture Patients in other hospitals with larger samples.
