Abstract

This research intended to contrast and evaluate an action of 3 intracanal medicament classical calcium hydroxide paste, Calcium hydroxide nanoparticle and Combined calcium hydroxide with silver nanoparticle , and their effect within necrotic root canals regarding to:
1- In vitro E.faecalis bacterial biofilm count reduction after application of 3 intracanal medicament within necrotic root canals.
2- In vivo bacterial count reduction after application of 3 intracanal medicament within necrotic root canals and pulp.
3- In vivo bacterial count reduction within dentinal tubules after application of 3 intracanal medicament in necrotic root canals and pulp.
4- Clinical evaluation of post-operative pain reduction after application of 3 intracanal medicament within necrotic root canals and pulp.
In part I of the study 45 extracted single rooted teeth were selected and splitted to three groups, after sterilization of samples and complete mechanical preparation. Microbiological culturing of E.faecalis was done and incubated for seven days. After incubation period, proper irrigation and premedication application sample was taken then application of intracanal medication was done. The samples were maintained in 37 oC for seven days. Second microbiological sample was taken after seven days and culturing on specific agar plates were performed to allow bacterial count reduction Results explained that there was significant difference among 3 groups.
In part II of the study, thirty three medically free patients with necrotic single rooted teeth were divided into three groups. After opening access cavity and completing mechanical preparation, proper irrigation and premedication application sample was taken then application of intracanal medication was done. After 7 days, patients recall to take post medication sample. All microbiological samples using sterile paper point and H file, they were cultured on different agar plates and counting of bacterial reduction were done for aerobic , anaerobic bacteria within necrotic root canal and within dentinal tubules. Results explained that there was a statistically significant difference between all three groups.
Part III in the study, thirty three medically free patients with necrotic single rooted teeth were divided into three groups. After opening access cavity and completing mechanical preparation, proper irrigation and premedication application sample was taken then application of intracanal medication was performed. also, Patients were instructed to record their postoperative pain level in pain chart (Numerical Rating Scale (NRS)) next to six, 24 and forty eight hours. After 7 days, patient recalls to take post medication sample and receiving pain chart evaluation consequences revealed that there was insignificant difference between three groups with 6 and 24 hours but there was a statistically significant difference among them next tor 48hours.
Conclusion:
The following conclusion can be derived from the results of this study:
1- Postoperative pain after 48 hours was better relived with using combination of Ca(OH)2with Ag NPS as intracanal medication than Conventional Ca(OH)2.
2- Positive correlation was found between the higher antibacterial activity of Ca(OH)2with Ag NPS as intracanal medication and pain reduction.
