Abstract

Background: Egypt has the highest prevalence of HCV infection worldwide, so the Egyptian MOHP has proposed a Plan of Action for Prevention, Care and Treatment of Viral Hepatitis in Egypt, 2014-2018, which included seven pillars. Objectives: This health system research aimed to assess the implementation of some pillars of the National Action Plan in different selected Hospitals in Cairo, and to assess the HCV patients’ satisfaction regarding care and treatment. Methods: The current study is a cross sectional study. It recruited 180 HCWs from Internal Medicine Departments and Blood Banks in University, MOHP, and Insurance Hospitals and they completed a questionnaire about IC, HBV vaccination and blood safety. Also, 10 nurses from each Hospital were observed for safe injection practices, 300 HCV patients from Viral Hepatitis Clinics in the three Hospitals were asked about their satisfaction with the services provided for HCV care and treatment using Hepatitis patients Satisfaction Questionnaire (HPSQ), and 15 managers from the three chosen Hospitals were interviewed. Results: HCWs and managers from University Hospital were more knowledgeable about the National Action Plan and its pillars than others from MOHP and Insurance Hospitals. HBV vaccine policy application in MOHP Hospital is more satisfactory than the other 2 Hospitals. The observed nurses from University and Insurance Hospitals have good practice for injection safety than nurses from MOHP Hospital. The total score for HCV patients’ satisfaction in the three Hospitals was significantly higher for the following items: HCV patients from University Hospital, illiteracy, non-skilled workers, no associated diseases, no DAAs side effects, and previous HCV treatment. Recommendations: Continuous medical education to HCWs especially in MOHP and Insurance Hospitals, HBV vaccine should be mandatory for all HCWs, increasing population awareness about HCV infection, and the importance of screening for HCV and early diagnosis.
