Abstract

Abstract : Asthma is known to be a heterogeneous disease which is a problem in the manangement. Symptom-based asthma phenotyping with endotyping of the proposed phenotype is a trial to solve this problem. Classification of asthma as regard to phenotypes and endotypes will facilitate researches, establish genetic associations, identify biomarkers and test for new lines of treatment. The aim of the Study: is to clarify the cytokine profile of wheezy asthma phenotype which can bave the way to personalize asthma medicines according to symptom based asthma phenotypes. A case-control study was conducted in (50) asthmatic patients presented solely with wheezes with Mean±SD of age was 9.54±2.81 years and (50) healthy controls with Mean±SD of age was 8.98 ± 2.79 years.The studied cases and controls underwent for assessment of serum levels of IL-10, TGF-β1, total serum IgE, serum peripheral eosinophilic percent as well as pulmonary function tests. Our study showed that Allergic rhinitis was present in 40% of wheezy asthmatic cases, meanwhile, (22%) of cases were suffering from atopic dermatitis. Regarding the inflammatory profile, they showed a significant increase in total serum IgE, peripheral eosinophilic percentage and TGF-β1. Whereas, serum IL-10 showed a significant decrease in asthmatic cases compared to controls. We found wheezy phenotype showed a significant increase of serum TGF-β1 levels as a promoting effect of airway remodeling, significant decrease in serum IL-10 and significant increase of both total serum IgE and eosinophilic percentage as a mirror image of a mixed pattern of airway inflammation.
