Abstract

The aim of this clinical audit study is to assess the degree of adoption of management of febrile neutropenia in non-malignant conditions to the implemented guidelines in Hematology unit in Assiut University Children Hospital (A.U.C.H). The target populations of this clinical audit study were all patients from 1month to 18 years admitted to hematology unit at AUCH during the period from 1st of April2018 to 31st of March 2019 (one year study).The study included 50 cases, 22 were males and 28 were females.10 cases were below the age of five years, 18 cases between the age of five years and ten years and 22 cases more than ten years old. Data of the study showed that AUCH partially followed the reference standard of the study. Data about the name, age and sex were recorded in 100% of cases. Data of history taking such as history of fever was fulfilled in 100% of cases, history of GIT and respiratory symptoms was fulfilled in 100%, history of oral mucosal lesions was fulfilled in100% of cases, history of similar family conditions was fulfilled in 100% of cases . Data about examination of the patients including general and systemic examination were recorded in 100% of cases with exception of perineal examination was done for 20% of cases and neurological examination was done for only 8%. Data about investigations done showed that CBC was done for 100% of cases ;all were abnormal showing mild neutropenia in 12% ,moderate neutropenia in 8% and 40% with severe neutopenia. CRP done for 48% and was positive in all of them, ESR was done for 24% of cases; all showed high ESR, kidney and liver function tests were done for most of the cases, blood cultures have been done for 72% of cases with abnormal findings in 32% , urine analysis and urine culture were done for some of cases ,stool culture was done for only 4% of cases showing no growth and lastly bone marrow biopsy was done for 92% of cases all of which were hypopcellular. Imaging studies were done for some cases such as chest x-ray done for 40% of cases with abnormal findings in only 4% and abdominal ultrasonograghy done for 68% of cases with abnormal findings in 22%.Data about management was recorded in 100% of cases including isolation, follow up charts, medications as antimicrobial agents, G-CSF and blood products transfusion. Our present clinical audit study concluded that AUCH partially follows the protocol of management of febrile neutropenia in children as regard: There’s defect in history taking from the studied cases. Full clinical examination had not been completely fulfilled for minority of cases. Some investigations which may have a role in management were not done for some of cases as blood cultures, CRP, ESR. Imaging studies were limited somewhat for our cases.
