Abstract

Acute respiratory tract infections represent a cause of great importance of under-five children’s morbidities and mortalities according to WHO annual reports. Aiming to control such problem among many other problems facing under five children IMCI program was established by WHO and UNICEF and was implemented in most countries around the world. Therefore arises the need for ongoing assessment of IMCI program application in different health facilities to ensure the best adherence to the program`s guidelines for diagnosis and treatment and consequently better outcomes. The aim of the current study was to assess the IMCI program application in management of respiratory tract infections in an Egyptian village. This study was carried out in 3 randomly selected health facilities on 246 under-five children with their mean age 24.5 months, 50.8% males and crowdedness index for 61% of them was ≤ 1. 90.2% received exclusive breastfeeding during their 1st 6 months of life and only 56.5% received vitamin D supplementation during the first year of life. 68.7% of the mothers were in their 20th. 69.5% & 70.7% of the fathers and mothers received secondary education respectively. 92.3% of mothers were unemployed housewives and 56.5% of the fathers were manual laborers or farmers.32.9% of the fathers were smokers and 88.9% of them smoke cigarettes. Regarding history taking and examination 4.9% of children were checked for history of convulsions while 86.2% of them were checked for ability to drink or breastfeed and 94.7% for vomiting. Checking for stridor or wheeze was the main focus for the physician during checking for cough which was done for 93.1% of all children. No child was referred. And as for drug prescription; only 17.1% of the examined children received antibiotic and its use was explained only to 33.3% of the mothers. As for other drugs; mucolytics, antitussives and antihistamines were the most prescribed drugs respectively (96.3%, 85.4% and 77.2%). Overall IMCI application assessment showed that 7.3% of all children received proper IMCI services that met more than 80% of its parameters while 51.2% received below average quality service covering less than 50% of the parameters. There was no statistically significant Association between IMCI assessment and child sociodemographic characteristics. Although there were a statistically significant difference in the provision of IMCI between the three health facilities (El-Hasaina, El-Salam, Parkin); EL-Hasaina & El-Salam have high risk of poor IMCI services compared to Parkin Unit (OR = 3.26 and 1.08, respectively).
