Abstract

The current study was prospectively performed in the Department of Tropical Medicine & Gastroenterolgy at AL-Rajhi University Liver Hospital, Assiut University in the period between June 2018 and January 2020. Cirrhotic patients with ascites who presented with fever, abdominal pain, abdominal tenderness, hepatic encephalopathy, renal impairment, leucocytosis, evidence of intra-abdominal source of infection, HCC and patients took antibiotic within last two weeks, were excluded from our study. Frequency of asymptomatic SBP in the current study was 15.7%(11 out of 70 patients).Mean age of those patients with asymptomatic SBP was 53.81 ± 15.34 years. There were no significant difference between asymptomatic SBP and non SBP groups regarding age, sex, residence and co morbidities. The etiology of liver cirrhosis of the majority of cases of both groups was HCV (42 out of 70) (60%).As regard laboratory studies: there was no significant difference between asymptomatic SBP and non SBP groups including complete blood count, liver function tests, coagulation profile , renal function tests, electrolytes, Child Pugh and MELD score. The prevalent variants in the current study showed classic SBP (PMNL≥ 250 cell/mm³ with positive culture) was presented in only one patient (9.1%). Culture-negative neutrocytic ascites (PMNL≥ 250 cell/mm³ with negative culture) was presented in 6 (54.5%) patients while monomicrobial non-neutrocytic bacterascites (PMNL< 250 cell/mm³ with positive culture) was presented in 4 patients (36.4%).Based on culture of ascitic fluid of those patients with positive growth, three patients were infected with staphylococcus (60%) while only one patient was infected with E.coli (20%), and also one patient was infected with acinetobacter (20%).Out of 5 patients with positive ascitic fluid culture, only 1 (20%) case was cefotaxime sensitive and 4 cases (80%) were cefotaxime resistant with sensitivity to other types of antibiotics including: ciprofloxacin (4 cases). We concluded that from our study: 1-Frequency of asymptomatic SBP in the current study is 15.7%.2-The SBP variants in the current study were the classical SBP (9.1%), CNNA (54.5%) while the bacterascites was (36.4%).3-The prevalent organism in culture positive cases in the current study was staphylococcus (60%).4-The current study showed that there is no significant predictor for asymptomatic SBP in patients with liver cirrhosis. 5- Ciprofloxacin was the most frequent antibiotic sensitive in the culture positive asymptomatic SBP in our study (4 out of 5).
