Abstract
Background: Postoperative pain is defined as pain of any degree after initiation of endodontic treatment either intra-appointment or post-obturation and is considered an undesirable occurrence for both patient and dentist. It was suggested that bacterial injury is probably the major cause of pain. Intra-canal medicaments are widely used to kill any bacteria surviving after instrumentation and irrigation. The aim of this study was to assess the ability of Asiatoside versus no dressing, Ledermix, corticosteroid /antibiotic combination used as intra-canal medication in reducing postoperative pain. Methods: 40 patients with apical periodontitis and necrotic pulps were randomly assigned into four groups according to the intra-canal medication used: Ledermix, corticosteroid and antibiotic combination, Asiatoside and no dressing. Preoperative pain was recorded using numerical rating scale,visual analog scale and verbal rating scale.After isolation, access cavity was performed followed by chemico-mechanical preparation using NiTi Protaper Universal rotary files with 2,5% sodium hypochlorite irrigation. Subsequently, intra-canal medication was placed and postoperative pain was recorded after 4 hours then daily for 4 days postoperatively.
Results: Pre-operatively; there was no statistically significant difference between mean pain scores in the four groups. After 4 hours and daily for 4 days, Asiatoside and ledermix paste showed highly significant results in reducing the inter-appointment pain followed by corticosteroid /antibiotic combination past and while no dressing showed the highest postoperative pain .
Conclusion Asiatoside is an efficient intracanal medicament for control postoperative pain comparable to Ledermix which is a gold standard pain controlling medication. Antibiotic /corticosteroid prepared past May considered an acceptable medication for reduction of postoperative pain.
