Abstract

Sexuality is an important and complex issue for quality of life and general well‐being, and contrary to general opinion, women with psoriasis and vitiligo are at increased risk for sexual problems. Female sexual dysfunction may be considered as a silent complication of some cutaneous diseases as psoriasis and vitiligo.
Sexual dysfunction can put a high psychological burden on patients with psoriasis and vitiligo, and it can have a negative impact on marital relations already burdened by the presence of a chronic illness.
This study was submitted on 50 females with psoriasis, 50 females with vitiligo and 50 normal females who were in stable continuous marriage. They did not have other factors rather than psoriasis or vitiligo that may affect their sexual function as gynecological, endocrinal problems, drug intake or chronic diseases or operations.
Females with any dermatological diseases except psoriasis and vitiliog were excluded. Pregnant, lactating females and those on contraceptive pills were also excluded. The husbands have fair general health and sexually active.
Full history was taken from every female and dermatological examination was assessed including type and severity of psoriasis by PASI score as well as type of vitiligo.
Sexual function of participating women was assessed by using Arabic translation of Female Sexual Function Index Questionnaire designed by Rosen (70). That evaluates sexual desire, arousal, lubrication, orgasm, satisfaction and pain domains.
In our Egyptian society, the attitude towards sex and talking openly about it-especially in females- is reaching the level of taboo. To the best of our knowledge none – in Egypt- has studied the issue of sexual dysfunction in psoriasis and only two published previous studies were done before discussing the issues of female sexual dysfunction in vitiligo patients.
So, we aimed by this work to evaluate the negative impact of common cutaneous diseases such as psoriasis and vitiligo on sexuality and the possible etiological factors living at Menoufia Governorate.
The results of the current study showed that there was a significant difference regarding the prevalence of sexual dysfunction between psoriasis and control groups and between vitiligo and control groups (with P value= 0.027 and 0.005 respectively). Studying the relation between age of psoriatic females and sexual activity revealed a significant relation (with P value=0.017).
The course and the duration of psoriasis were significantly associated with sexual activity (with P value= 0.001 and 0.013 respectively).
There was a highly significant relation between PASI score, PASI grade and the patient concept about affection of psoriasis to sexual relation and sexual activity (with P value <0.001, <0.001 and 0.001 respectively).

Also, the duration of marriage, age of husband and type of vitiligo showed a significant relation with sexual activity (with P value= 0.023, 0.016 and 0.011 respectively). While, the course of vitiligo and the patient concept about affection of vitiligo to sexual relation were highly significantly associated with sexual activity (with P value= 0.001 and 0.001 respectively).
Regarding sexual dysfunction domains among studied groups, there was a significant difference between psoriasis and control groups regarding the frequency of intercourse, orgasm, satisfaction and total function index domain scores (with P value 0.015, 0.021, 0.001 and 0.008 respectively).
Also, there was a significant difference between vitiligo and control groups regarding lubrication, arousal, satisfaction and total function index domain scores (with P value 0.035, 0.007, 0.001 and 0.001 respectively).
While, there was no significant difference among studied groups regarding desire and pain domain scores.
There was no significant relation regarding sexual activity between patients with psoriasis and vitiligo alone or those with associated comorbidity.
