Abstract

This prospective study was conducted on 15 patients with lumber spondylolysis treated by pedicular screw with laminar hook system in Minia University Hospital from October 2017 to January 2019.
Informed consent was obtained from the patients following the rules of our local medical ethical committee.
Patients were followed up regularly at 2 weeks, 3m, 6m, 9m and at the end of the study respectively to evaluate the results clinically, radiologically, functional outcome and complications.
The data collected from patients was analyzed, evaluated, and the observations were recorded. The aim was to evaluate the results of pars defect reconstruction by pedicular screw rod laminar hook system .
All the patients were examined clinically, neurologically and radiological, including detail history of trauma and duration of low back pain at the time of admission. Patients fulfilling the inclusion criteria were only included in the study.
The study included 15 patients ( 5 males , 10 females ) . Mean age was (26.47 +6.97) ranging from 15 to 40 years . There were female predominance ( 66.7%).
The most affected level of spondylolysis in this study was L4
( 60%) . Isthmic spondylolysis was the most common presented type of spondylolysis among our patients with percentage ( 46.7% ) .
Regarding disc status, the most presented grade of disc changes according to pffirman classification was grade one(53.3%) . Only three patients were associated with grade one spondylolithesis ( 20%).
Preoperative radicular lower limb pain was found in two patients (13.3%) and preoperative hamstring tightness in two patients (13.3%).
Associated injuries were found in two patients (13.33%), one of them was associated with fracture L1, fracture dislocation of Rt. shoulder , Lt. tibial plateau fracture and Rt. tibial eminence fracture , the other patient was associated with fracture T11 vertebra ,Rt. neck femur fracture and ipsilateral fracture shaft femur .
In this study, clinical and radiological assessment were done at each follow up. Mean union time was 4.5 months with range (4-6 months ).
The mean ODI value changed from a mean of 47.00±9.22 preoperatively to a mean of 5.67±1.76 at final follow up with patients satisfaction . The mean VAS value changed from a mean of 6.27 ±0.80 preoperatively to a mean of 1.20±0.41 at final follow up with patient satisfaction. The mean modified Prolo score value changed from a mean of 9.87 ±3.62 preoperatively to a mean of 19.47 ±0.74 at final follow up.
Hamstring tightness was improved postoperatively while the radicular lower limb pain was improved in one patient .
Results of Macnab criteria were excellent in 10 patients (66.7 %) and good in 5 patients (33.3%) with patients satisfaction. Patients return to activity within 4 to 6 months with mean 5.47±0.92.
Complications were found in 2 patients , one of them developed postoperative lower limb radicular pain on one side due to medial breaching of the pedicular screw on the canal and it was revised 10 days later and the position of the pedicular screw was corrected and she was relieved from radiculopathy, the other patient showed superficial wound infection.
