Abstract

Diabetic macular edema (DME) is the most common cause of visual impairment in diabetic patient. Intravitreal injection of drugs has shown good results in the management of DME. NSAIDs have shown a proven role in reducing the occurrence and severity of cystoid macular edema. This prospective randomized clinical trial included 45 eyes of 45 patients presented with established DME, they were 23 males 51.1 and 22 females 48.9 . All patients were diabetic; 31 patients were IDDM (68.9 ) while 14 patients were NIDDM. All patients received intravitreal injection with diclofenac Sodium. Diclofenac Sodium 500 µg/0.1mL (Voltaren ampoules) was used. It is diluted to obtain the desired concentration; 1 ml voltaren diluted in 4 ml saline, then take 0.1mm by a 27-gauge needle. Injections were performed under strict sterile conditions. Intravitreal injection was performed through pars plana, in the inferotemporal quadrant 4 mm posterior to limbus in phakic eyes and 3.5 mm posterior to limbs in pseudo phakic using a 27-gauge needle. After injection a topical antibiotic eye drops and ointment was applied and eye patching overnight. All patients were followed up at one day, one week and 4 weeks after IVI. By OCT evaluation of CMT of studied group before and after injection show that mean SD of pre-injection CMT 349.8±53.5 (range from 260 to 425µm), mean SD of 2weeks post injection 314.6±50.5 (range from 230 to393µm), mean SD of 1month post injection 293.9±49.7and mean SD of 2weeks after 2nd injection 317±51.3 (range from 226 to 389µm). The mean pre-injection IOP of studied group was ranged from 13.8 to 19.6 mmHg with median 16.5 mmHg and a mean of 16.8 and post-injection IOP ranged from 12.6 to 16.5 mmHg with median 15.1 mmHg and a mean of 16.4. P value was 0.001(significant). The post-injection complication in the studied group revealed that 27 patients have no complications by percentage 60%. Corneal erosions were found in 9 patients by percentage 20%. Subconjunctival hemorrhage was found in 9 patients by percentage 20%. There is no evidence of increase IOP.
