Abstract Mitral PPM is actually not a new concept; it is present when the effective orifice area of the inserted prosthetic valve is too small in relation to body size, which results in an increased postoperative transvalvular pressure gradient as identified in 1978.Previous studies reported that PPM after aortic valve replacement is quite frequent (20% to 70%) and associated with worse hemodynamics, left ventricular hypertrophy, more adverse cardiac events and a major impact on short-term mortality, particularly if leftventricular dysfunction is present, and it has a moderate impact on long-term mortality. On the other hand, mitral PPM remained unexplored for a long time and might have been thought to be a relatively rare phenomenon with minimal impact on postoperative outcomes and mortality. |
