Abstract

Knowledge the anatomy of left coronary and right coronary arteries is of great importance to avoid misdiagnosis of coronary illnesses and for proper stenting during percutaneous coronary intervention. Acquaintance of the variations of the coronary arteries is significant for assessment of coronary insufficiency. Three dimensional CT coronary angiography (3D-CCTA) and coronary catheterization angiography provide high-quality images and allow assessment of the anatomical variations of the both coronary arteries.
The present study was designed to assess the incidence of different anatomical variations of the LCA, RCA and their branches. It was carried out on thirty human adult cadaveric hearts , two hundred 3D-CCTA scans and two hundred coronary catheterization angiography obtained from Anatomy , Radiology and Cardiology Departments, Beni Seuf ,Cairo and Minia Universities.
The current work revealed that there wasn’t any case of higher take off origin of left coronary artery .All of them originated from the left posterior aortic sinus. While the incidence of high takes off origin of right coronary artery was 0.23% of all studied hearts.
The incidence of the LCA branching mode was 74.1%, 24.18% and 0.46% for the bifurcation, trifurcation and tetrafurcation; respectively.
Moreover, statistically significant relation was detected between the incidence of the diagonal branches of the LAD artery and the mode of branching of the LCA. On the contrary, nonsignificant correlation was found between the presence of the obtuse marginal branches of the CX artery and the branching patterns of the LCA.
In 13.04% of the studied hearts, myocardial bridges had been recognized across the course of the LAD and its branches .Statistically significant relation was detected between the incidence of the myocardial bridge and the branching pattern of LCA.
The incidence of coronary artery dominance was 77.4%, 13.5% and 9.1% for right , left and co-dominant pattern.
It can be concluded that a perfect knowledge of the anatomical variations of the mode of branching of the LCA and RCA is essential for cardiologists and radiologists, during diagnostic and therapeutic cardiac procedures.
