Abstract

I
n our hands, Arthroscopic repair of Bankart lesion is an effective primary treatment for Anterior shoulder dislocation, it provides successful outcomes without high rate of recurrence of instability. The reattachment and tightening of the torn labrum into the glenoid bone done by using a pull-out suture anchor with a sliding knot to tighten the detached labrum into the glenoid bone. This is a technically demanding procedure that requires relatively long learning curve for the patient position and expecting the exit of the passing pin.
Correct indications, careful preoperative evaluation and good dissection and reduction of anterior glenoid labrum are the keys for successful long-term outcome. The superiority of this technique over others can only be established in an adequately powered randomized controlled trial with long term follow-up.
Our technique does not preclude further surgical procedures when failure occurs, and preservation for glenoid bone stock is achieved.
We stress that long-term evaluation is necessary, particularly to monitor the possible development of glenohumeral osteoarthritis and the long-term functional deficit that recurrent shoulder dislocation may occur.
Advantages
1- Avoidance of metal and biodegradable anchors complications.
2- Anatomically anterior glenoid labrum repair by sliding knot.
3- Cost savings.
Disadvantages
1- Long learning curve.
2- The possible supra scapular nerve injury.
3- Sutures tunnels convergence during drilling.
