Abstract To compare the results of two different frontalis sling designs for severe congenital ptosis: Fox’s single pentagon and a single triangle technique as regards cosmetic outcome, functional success. Methods: In comparative prospective study included 37 eyes of 30 patients with simple severe congenital ptosis and poor levator function (≤4 mm); divided to two groups; group I underwent Fox’s single pentagon technique (15 patients with 17 eyes), and group II underwent single triangle technique (15 patients with 20 eyes). Cosmetic outcome, and functional success. Results: follow-up time of 6 months (range, 6.0 – 8.0 months). The patients achieved excellent cosmetic outcome in both groups. The intergroup difference was not statistically significant (P > 0.05). However, single triangle patients had better cosmetic outcome as regard scare evaluation with significant intergroup difference (p < 0.05). The mean functional success in Fox’s single pentagon group is 82.4% and in single triangle group is 90%, which indicated that both techniques had same effect on functional success as majority of patients in both techniques were succeed functionally. Mean increase in marginal reflex distance 1 (MRD-1) 3.3± 0.14 mm in the Fox pentagon group and 3.6 ±0.18 in single triangle group. The difference in the groups was not significant. Conclusion: Both techniques were effective in reducing the amount of blepharoptosis and achieving a natural appearing cosmetic outcome. Based on the eyelid crease incision, optimal exposure of tarsus and tarsal sling fixation principles. The single triangle design using the open technique was recommended as simple, effective with better cosmetic outcom. |
