Abstract Inguinal hernia is the most common type of hernias; More than 70% of all hernia that occur are inguinal type. For practical purposes inguinal hernias have been classified into indirect and direct types. Clinically they may be asymptomatic, reducible, irreducible, incarcerated or painful, obstructed and strangulated. The exact cause of inguinal hernia is still unknown but the following factors contribute in its occurrence. A congenital sac, raised intra abdominal pressure and weak abdominal musculature. There is no ”gold standard” operation for treatment of inguinal hernias. The optimal surgical approach must be selected individually for the patient, taking into account patient age, hernia size, unilaterality or bilaterality, primary or recurrent status, and type of anesthesia, occupation, and patient activities. There are three important landmarks in the history of repair of inguinal hernia: 1. Tissue repair (Eduardo Bassini 1889). 2. Onlay mesh (Irving Lichtenstein1984), tension-free repair. 3. Laparoscopic hernia repair (1990). Laproscopic repair of inguinal hernia includes both TAPP and TEP approach. The aim of this study is to evaluate a new technique (Zein method) of mesh fixation using sutures in laparoscopic inguinal hernia repair, as regards to operative data, cost, postoperative pain, postoperative complications and recurrence rate; and compare results with those of stapled mesh fixation from literature. This study will be conducted at General Surgery Department, Menoufia University, and it will include 50 patients who are diagnosed as having inguinal hernia as diagnosed clinically and by ultrasonography. Ethical committee agreement will be taken. Informed consent from all patients will be taken. All the two groups were compared for: Operative duration. Intra-operative complications. Postoperative pain. Postoperative hospital stay. The mean time until return to work. Postoperative complications. Our results showed that the age of studied patients ranged from 27 to 55 years with mean 42.82±7.90 age/years. Regarding sex of studied patients, more than half (74%) of studied patients were males and (26%) were females and the time of operation ranged from 32-45 minutes with mean 40.06±3.260 minutes. While, Post-operative pain ranged from 1 to 3 patients with mean 1.90±0.36 on visual analogue score This study confirmed that all of the studied patients had no Seroma or Infection. On the other hand, 2% of the studied patients had Hematoma and 98% had not Hematoma, also most of the studied patients hadn’t recurrence of disease after 3 and 6 months except 1 patient (2%) had recurrence after 6 months Finally we noticed that the Hospital Stay of Studied patients ranged from 1 day to 6 days with mean 1.52±0.81 day. Regarding Antibiotic use ranged from 4 to 7 with mean 6.48±0.76.Our results also showed that the Overall Cost of Studied patients ranged from 2100-2600 Egyptian pound with mean 2096±30.260 Egyptian pound. |
