Abstract

The principle of free-style perforator-flap surgery provides greater freedom in choosing a donor site location, because flap selection is based on the quality and volume of soft tissue needed at the recipient site.Flap design and harvest are carried out according to previous Doppler mapping. Patients and Methods: A prospective interventional descriptive pilot study was conducted at Surgical Unit of Oncology Centre Mansoura University (OCMU) in the period from from 4/2017 to 10/2019. After obtaining patient consent, 24 patients met the criteria and underwent wide local excision of facial cutaneous lesions and reconstruction by free style local perforator flap. Reults and Discussion : One stage procedure is preferred and consideration to restoration and good preservation of facial features is crucial. Free style local perforator flaps were chosen in our study as a reconstructive choice for facial defects requiring one-stage procedure, whereby tissue matching the face’s color and texture is harvested. To meet these demands, different perforator flaps were used for facial defects in the present sample.  Conclusion : Freestyle perforator flaps can be based on one or more perforators obtaining a reliable blood supply together with great flexibility in design, free choice of orientation, arc of rotation up to 180°, wider range of motion compared with local flaps, and primary closure of the donor site along the relaxed skin tension lines to minimize any adverse scarring.  The free-style concept has significantly increased the potential and versatility of the standard local and recycled head and neck flap alternatives for moderate to large defects, providing a more robust, custom-made, tissue-sparing, and cosmetically superior outcome in a one-stage procedure, with minimal donor-site morbidity.  free style local perforator flap is safe and feasible technique in reconstruction of Small and medium sized facial cutaneous defects.  This technique can be safely done in Old age patients and patients with one or more co morbidities.
