Abstract Key words; ketamine, one lung ventilation, hypoxic pulmonary vasoconstriction, anti-inflammatory effect. The aim of the present study was to evaluate the protective effects of ketamine infusion on the lung during OLV in the thoracic surgery more specifically on oxygenation and anti-inflammatory effect. This study was conducted over 50 patients who randomly allocated into two equal groups by the closed envelop method. group A in which ketamine was used for maintaince of anesthesia and group B in which isoflurane was used for maintaince of anesthesia. This single blind randomized prospective controlled study was conducted at Mansoura university hospitals after approval from the institutional research board. All the patients included in the study were interviewd & Written and verbal consent were obtained. Patients of either sex who aged 20-60 years old, American society of anesthiologists II, III and underwent an elective thoracic surgery using one lung ventilation were concluded in the study. Exclusion criteria included Patient refusal, forced expiratory volume1 FEV1 50%, chronic liver disease Child B&C, chronic renal disease serum creatinine 1.5, respiratory failure, heart failure or contraindication to the study drug. We can conclude that Continuous infusion of ketamine has no effect on oxygenation, there was no difference in PaO2 and shunt fraction in comparison with isoflurane but has significant anti-inflammatory effect as ketamine group has lower levels of CRP compared to isoflurane. |
