Abstract

Despite advances n diagnostic modalities especially diagnostic radiology, Traumatic diaphragmatic injury remains one of the difficult diagnostic dilemmas in medicine because of its silent nature ragarding symptoms or signs either clinically or by investigations and it rarely express itself in absence of organ herniation which may occur late and cause morbidity and mortality .
In this study we presented cardiothoracic and general surgery departments Sohag university hospitals expeience in diagnosis and management of missed traumatic diaphragmtic injury. This is a retrograde descriptive study of 27 patients diagnosed with missed traumatic diaphragmatic injury in cardiothoracic and general surgery departments in the period between January 2015 and January 2018.
Patients were studied according to their demographic data, data of previous injury, data of recent pesentationa and investigations, and data of surgery.
TDI predomonently affected middle aged males and left side was the main side of injury.
Dyspnoea was the most common presenting symptom followed by abdominal pain and manifestations of intestinal obstruction.
Multislice CT was the most sensitive and specific radiological investigation
Thoracotomy was the preferred approach as it gives good chance for dissection of adhesions between abdominal and thoracic organs, laparotomy is preferred when complaint is only abdominal with no respiratory symptoms as this indicates little adhesions.
Morbidity and mortality of missed TDI is not little due to intestinal obstruction and respiratory distress.
