Abstract

Preeclampsia/eclampsia syndrome is one of the life threatening disorders encountered during pregnancy, it doesn’t affect only the maternal and fetal life but also it carries a great impact on their morbidity due to the accompanied complications in the form of eclamptic fits, multi- organ failures as renal and liver failure, Cerebrovascular strokes, pulmonary embolism and DIC.
Due to its impact on health status, it is necessary to provide different facilities to predict the development of that disease in order to prevent its complications via identification of high risk group, administration of high dose aspirin and frequent periodical monitoring of those at risk.
As the hematological changes are evident in PE pathophysiological mechanisms due to generalized inflammation and endothelial dysfunction, the routinely requested complete blood count can be used for prediction of PE by utilization of its different parameters as increased white cell count and red cell distribution width as well as various platelet parameters.
Although the pathophysiology of PE is still investigated, the most accepted theories link the disease occurrence to presence of generalized endothelial dysfunction that increase the destruction and consumption of platelets when coming into contact with the injured endothelium leading to progressive decline in platelet count. This decline stimulates the bone marrow to increase the production of platelets to balance the wasted platelets. Although this compensatory mechanism is important, the produced platelets are still immature with larger sizes and higher tendency to be activated.
These proposed changes in platelet indices occur as early as 10th to 12th week of gestation before appearance of disease symptoms and can be detected in CBC to help in monitoring the disease development and progression.
This study aimed to investigate platelet parameters and compare them between two groups of pregnant women in the third trimester (n=120), first group included 60 healthy normotensives (n=60) and the other group included another 60 women with preeclampsia (n=60); 30 women with mild PE and the other 30 women had severe PE.
The results showed that there were statistically significant differences between the cases and controls as regard the following parameters: Platelet count, MPV, PDW, PLCR and platelet count to MPV ratio and all of these parameters can be used to predict the development of PE.
