Abstract

This study shows how ultrasound guidance allowed the performance of both blocks without increasing the dose of local anesthetic, allowing this technique to be used as a sole anesthetic technique in procedures involving the entire upper limb instead of general anesthesia (GA). In this study sixty patients of ASA III, IV aged (21_ 60) years old of either sex with chronic renal failure who were scheduled for brachioaxillary graft insertion and divided into two groups: GA group who had general anesthesia, combined block (CSIB) group who had combined ultrasound guided SCB and ISB. This study proved that using combined interscalene and supraclavicular approach to block brachial plexus was found to be effective as a sole anesthetic technique for brachioaxillary graft insertion for dialysis with no need to increase local anesthetic dose and less incidence of complication. It is of great importance in patients where general anesthesia is a great risk.
