Abstract

This prospective, comparative, single blinded study was conducted on 60 pregnant women presented with IUGR. The study comprised two groups; Sildenafil group received sildenafil citrate 20 mg orally twice daily while Aspirin group received acetyl salicylic acid 81mg orally once daily. Both groups were followed using Doppler velocimetry on uterine and umbilical and fetal middle cerebral arteries was be performed at 18,28,34 weeks.Sildenafil was associated with a significant decrease in the mean arterial blood pressure as it causes vasodilates the myometrial arteries isolated from women with IUGR-complicated pregnancies. Such change was not seen in the aspirin group.Regarding the fetal growth measurement (biparietal, head circumference, abdominal circumference, estimated fetal weight and femur length) after both sildenafil citrate administration in the first group and aspirin in the second group. This indicates a significant increase in placental perfusion evaluated by changes in umbilical artery Doppler and middle cerebral artery Doppler where significant changes in Doppler indices occurred after treatment.Doppler blood flow results of the studied arteries (uterine, MCA and umbilical arteries) showed significant improvement in the readings of umbilical arteries RI and PI in both Aspirin and Sildenafil groups however this improvement was nearly similar in both groups It is worth to mention that, mean birth weight at delivery was increased in both groups; however, it was more in sildenafil group compared to acetyl salicylic acid group The current study findings proved that both Sildenafil citrate and acetyl salicylic acid administration had effective outcomes on fetal growth parameters as well as improving utero-placental perfusion.However, there is some data reporting undesired effects of Sildenafil citrate such as headache, visual disturbances and gastrointestinal tract symptoms. Moreover, in some studies, some cases stopped sildenafil due to its side effects.
