Abstract

The duplex scanning offers multiple unique features that have manifested into the use of duplex imaging for guidance in percutaneous transluminal angioplasty. The implementation of this exciting and novel technique approach for endovascular interventions proved to be viable, safe, and effective.In this study we tried to evaluate the technical success of duplex guided angioplasty for treatment of femoro-popliteal arterial occlusive diseases.This study included 25 patients with femoro-popliteal ischemic lesions that were revascularized under duplex guidance during the period from March 2017 to April 2019 in a prospective manner.After full history taking, thorough general and local examination including assessment of the degree of ischemia, investigations including ABPI, then duplex study of lower limb arteries were done for all patients.After that, endovascular revascularization of the lesions was done under duplex guidance. Post-operative reevaluation of ABPI,post-operative duplex scan was done The following data were collected and registered for each patient at time of admission, immediate post-operative, at 1 week follow up and at 1 month follow up: ABPI at anterior and posterior tibial arteries,PSV at previous lesion site as well as distal to the lesion, waveform at and distal to the previous site of lesions.These data were compared between different follow up periods,and between similar studies.The technical success of duplex guided angioplasty for femoropopliteal arterial lesions was achieved in most of our cases (84%), also it reached 100% in other similar studies.There was both clinical improvement (improvement of claudication distance, healing of amputated gangrenous toes, or healing of resistant ulcers). Also there was hemodynamic improvement regarding ABI, PSV, waveform).Our results were consistent with those of other studies that ensured safety and efficacy of this technique in treatment of femoropopliteal arterial occlusive diseases
