Abstract This study was carried out to evaluate the early outcomes of deep anterior lamellar keratoplasty and factors affecting it. The study included 20 eyes undergone DALK performed with a modified techniqueescematic layer is done only in the , using large diameter 9 mm graft prepared with microkeratome (400 um thickness blade) with debulking of the anterior stroma to the trephination depth in the whole 9 mm but baring of the predcentral 6 mm using the Anwar’s big bubble or manual scissor dissection when big bubble couldn’t be achieved. Fifteen keratoconic eyes and 5 eyes with stromal opacities were included. Pneumatic dissection was successful in 80% of eyes. The overall rate of DM perforation was 6/23 eyes with attempted DALK, 3 of them were converted to penetrating keratoplasty and excluded from the study. Visual acuity improved in all cases with 90% of cases achieved 0.5 or more at 6 months postoperative. Mean endothelial cell loss was 16.7% at 6 months. Mean KA was 4.13 ± 3.35 D at 6 months. Fourier analysis showed significant decrease of asymmetry at 3 months and 6 months post operative which correlates to the improvement in BCVA. |
