Abstract

Vitamin D is a fat-soluble vitamin. It is important for calcium homeostasis and ensures adequate calcium supply for bone mineralization by its effect on bone, kidney and gut. However, vitamin D has pleitropic effects including cellular proliferation, differentiation and immunomodulation. These extraskeletal effects are related to the pathogenesis and treatment of infections, cardiovascular ,autoimmune , degenerative diseases and multiple types of cancer . Vitamin D is biologically inactive and need to be hydroxylated in the liver then in the kidney to l,25-dihydroxyvitamin D. So,the liver is critical to the metabolic activation of vitamin D.Several studies showed vitamin D deficiency is highly prevalent in CLD. That was explained by poor nutrition, defective metabolic activation of vitamin D by liver ,low sunlight exposure, decreased levels of carrier proteins such as albumin and vitamin D-binding protein , increased vitamin D catabolism, decreased intestinal vitamin D absorption due to interruption of enterohepatic circulation.It has been also reported that the incidence of vitamin D deficiency increases as the liver disease progresses. Many clinical applications of 25(OH)D levels have been suggested, including its use as a non-invasive marker of liver fibrosis in chronic hepatitis C. Our study was done on 25 HCV cirrhotic patients (compensated and decompensated) with 25(OH)D level below 30 ng/ml recruited from inward and outpatient clinics of internal medicine department, Tanta university to evaluate the effect of vitamin D replacement therapy along 3 months on laboratory parameters in HCV cirrhotic patients
