Abstract

Sacrococcygeal pilonidal disease is a common problem especially among adolescents. It is believed now, according to theories to be an acquired condition aggravated by predisposing factors, rather than of congenital origin. Pilonidal disease has various clinical presentations with no clear consensus on the optimal treatment. Techniques which involved the use of a flap procedure have been developed specially for complex and recurrent cases which present a challenge to the surgeon.
Our study was designed on 40 adult patients with complex or recurrent pilonidal disease, to compare between MLF reconstruction, a commonly used flap technique and CL procedure regarding the short term outcome. They were randomly divided into two equal groups.
The CL procedure involved excision of skin from one side of the natal cleft with undermining the skin from the opposite site with off-midline closure. The MLF reconstruction involved a rhomboid-shaped incision containing sinus tracts with the Inferior apex lateralized 1–2 cm from the midline. The rhomboid flap is then transposed medially to fill the defect.
In this current study 57.5% (n=23) of patients were males and 42.5% (n=17) were females. There were no significant differences between the two groups in terms of demographic data.
The patients were followed up post operatively for up to one year. The study results showed significantly longer operation time for MLF in comparison to CL procedure, in addition to significantly higher patient satisfaction at day 30 for the CL group. However, other study results regarding hospital stay, post-operative pain, time of suction removal, post operative complications, healing rate and recovery time as well as incidence of recurrence showed insignificant differences between the two groups
