Abstract

Sleep is an important aspect in life that affects physical
health, quality of life and overall well-being in diseased as
well as healthy populations. Sleep and circadian rhythm
disturbances are prevalent in frail elderly
Frailty is a geriatric syndrome indicating multisystem
impairment and increased vulnerability he frailty phenotype in
the elderly is characterized by the presence of at least three of
the following items: decreased grip strength, a report of fatigue
and exhaustion, decreased activity, slow gait and weight loss,
Grip strength and slow gait speed are also parameters included
in the sarcopenia diagnostic criteria. Frailty and sarcopenia
share many risk factors, pathophysiological pathways and
clinical consequences.
The aim of this work is to study the prevelance of sleep
disturbances among frail elderly patients.
The study enrolled 100 frail elderly, divided to 3 age
groups 60-69, 70-79 and over 80 All participants underwent
medical history and clinical assessment,are screened by frail
scale and Clinical frailty index divided to mild, moderate and
severe frail Sleep disturbances were detected using the
Pittsburgh sleep quality index using Arabic version, assessment
of mood by the geriatric depression scale (GDS), assessment of physical function using activities of daily living (ADL) and
instrumental activities of daily living (IADL),and assessment of
nutrition done using Mini Nutritional Assessment(MNA).
Our study concluded that sleep disturbance is more
frequent among participants with frailty and that by increasing
the severity of frailty
All the component of Pittsburgh sleep quality index in
are affected sleep quality, sleep latency, sleep efficiency and
daytime dysfunction. Has significant relationship with frailty
Sleep Quality (component 6 in PSQI) is also prevalent in
frail but more in the mild frail group
Also in our study we concluded that there is a significant
relationship between comorbidities and sleep disorders
Heart failure showed significant relation between both
frailty and sleep disorders
Chest diseases also showed a significant relation with
sleep disorders
Polypharmacy (use of multiple medication) showed is
more prevalent in frail elderly, and more prevalent with frailty
as regards nutritional assessment, the frail elderly showed more
risk for malnutrition and malnourishment, and showed a
significant relationship between total Pittsburgh sleep quality
index score and malnutrition.
