Abstract SUMMARY Cardiac surgery has certainly advanced over the years, particularly with the development of the cardiopulmonary bypass machine. The CPB machine itself has come a long way in development since the first surgery and will continue to develop to enhance every patient’s quality of life. Although the CPB is an amazing machine, many adverse effects can occur. A systemic inflammatory response is the most common due to the blood being introduced to an entirely new environment. This inflammatory response can lead to many other problems throughout the body and ultimately organ failure may occur. Preventing these adverse effects is an ongoing process and will continue to be so. Using membrane oxygenators and adding filters throughout the machine have been useful in preventing microemboli from travelling throughout the body. Research has shown that coating the circuit with heparin may reduce inflammation, bleeding, the need for blood transfusions, and pulmonary injury. Corticosteroids are also identified to be beneficial in reducing cardiac complications and reducing the inflammatory response. This study was held in Ain Shams University hospitals. It included 50 patients equally divided into two groups. group A that received 1 gram solu-medrol and group B that received a placebo. The study was tracing the level of IL10, cardic enzymes, liver enzymes, kidney function, time of extubation and ICU stay. The study was comparing the results of the two groups. The results in general was favourable in group A, the level of IL10 was significantly elevated indicating the limitation of the inflammatory reaction. Thus the use of a single dose 10mg/kg of methylprednisolone before intiation of CPB in the CABG surgeries, proved to raise the level of anti-inflammatory mediator IL 10 in a sufficient level that attenuates occurance of inflammatory reaction. In addition this dose of methylprednisolone has proved to decrease the elevation of cardic enzymes signifying that it prevents the myocardium injury that may occur. Another beneficial outcome was the decreased ICU stay. |
