Abstract

The current study has been designed to know predictive value of MUST in comparison with NUTRIC score in critically ill elderly patients in geriatric ICU to predict unfavorable clinical outcomes (sepsis, long hospital stay, mechanical ventilation and death)
Ninety patients in the current study were recruited from Geriatric intensive care units (GICU) of Ain Shams University Hospitals and were assessed for malnutrition in the first 24 hours following ICU admission using NUTRIC and MUST scores then followed up in prospective study till death or discharge from ICU from October 2017 till June 2018.
Mean age in study population was 70.2±8.6 years, male patients constituted 46.7% while female patients constituted 53.3% with charlson co morbidity index 6.1±2.3 and main cause of admission was neurological disorder (21.1%) followed by metabolic disease (18.9%).
In the current study, it was found that albumin level showed no significance regarding malnutrition (p= 0.221).
Moreover, It was found that the percentage of the high risk group with NUTRIC score and MUST score were 11.1% and 16.7% respectively.
However, it was also found that MUST tool have low diagnostic performance and characteristics in prediction of NUTRIC high risk. The sensitivity of MUST tool was 80% and specificity was 67.5% in comparison to modified NUTRIC score which was 100% and its specificity was 87.5%.
The median APACHE II score for studied patients was 24.0±8.3 and the median SOFA score was 6.1±3.2. It was found that SOFA and APACHE II scores were significantly higher in cases at high risk for malnutrition by NUTRIC score
Additionally, it was also found that NUTRIC& modified NUTRIC scores were significantly higher in non-survivors and mechanically ventilated cases in contrast to MUSTscore.
On the other hand, in the current study it was found that there was no significant difference between septic and non-septic cases regarding nutritional scales.
Moreover, it was found that there was no significant negative correlation between ICU duration of stay and different nutritional scales.
In the present study, only age had a significant correlation with ICU stay duration although with a low coefficient of determination.
