Abstract

The present study was conducted to assess the relationship between nurse managers’ level of organizational citizenship behavior and staff nurses’ level of structural empowerment at Menoufia University hospital at Shebin El-Kom. This aim was achieved through assessing nurse managers’ level of organizational citizenship behavior, assessing staff nurses’ level of structural empowerment and assessing the relationship between nurse managers’ level of organizational citizenship behavior and staff nurses’ level of structural empowerment in the study setting.• The study was conducted at Menoufia University hospital at Shebin El-Kom including all 11 intensive care units, all 7 operating rooms, all 18 inpatient departments (emergency, oncology, surgical, physiotherapy,burn, cardiac and chest, hemodialysis (pediatric and adult), hematology,specific medical (male and female),cardiac, chest, obstetric, pediatric,ophthalmology, orthopedic, endoscopy, neonatal, urinary, poisons, cardio- catheterization, Ear, Nose and Throat, sterilization, infection control, and economic department) and 1 outpatient clinics department.• A pilot study was conducted after development of the instrument and before starting the actual collection of data. It was conducted to assess instrument clarity and applicability. It had also served in estimating the time needed for filling the instrument. The study was tested on 10 % of total subjects (37) staff nurses and (9) nurse managers to evaluate the feasibility and clarity of the instrument. The pilot study subjects included in the study sample because there weren’t any modifications in the instruments of data collection.Data for this study were collected by using two different instruments developed by the investigator after reviewing the related literature. The first instrument was organizational citizenship behavior questionnaire administered to nurse managers to assess their level of organizational citizenship behavior. The second instrument was structural empowerment questionnaire administered to staff nurses to assess their level of structural empowerment.• Instruments were tested for validity and reliability and administered to subjects. The instruments were distributed to a Panel of five Assistant Professors in the field of Nursing. Period lasted from the beginning of July month at 2018 to 8-8-2018. Cronbach’s alpha for organizational citizenship behavior scale was 0. 90 and structural empowerment scale was 0.87.The results of the present study revealed that:▪ The nurse managers’ levels of organizational citizenship behavior were high.▪ A highly positive statistically significant correlation was found between nurse managers’ levels of all dimensions of organizational citizenship behavior.
▪ The staff nurses’ levels of structural empowerment and its dimensions were high.▪ A highly positive statistical significant correlation was found
between staff nurses’ levels of all dimensions of structural
empowerment in the study sample.
▪ The nurse managers’ levels of organizational citizenship behavior
increased along with increased nurse managers’ age.A highly statistical significant correlation was reported between nurse managers’ levels of courtesy and their age, qualification and years of experience.▪ A highly statistical significant correlation between nurse managers’levels of altruism and their age and years of experience. The nurse managers’ levels were the highest of conscientiousness and altruism and were moderate of civic virtue.▪ The staff nurses’ levels of structural empowerment increased along with increased staff nurses’ experience years.▪ The staff nurses’ levels were high of access to information and opportunity, more than two thirds of informal power and lower of access to resources, support, formal and informal power.▪ There was a statistically significant correlation between staff nurses’ levels of access to information and staff nurses ’age and years of experience. Also, it illustrated statistically significant correlation between staff nurses’ levels of access to support and academic qualification of staff nurses. Furthermore, it showed a highly statistical significant correlation between staff nurses’ levels of formal power and staff nurses’ years of experience.▪ There was a highly positive statistical significant correlation between nurse managers’ levels of civic virtue and staff nurses’levels of all structural empowerment dimensions. Also, there was a positive statistically significant correlation between nurse >managers’ levels of altruism and staff nurses’ levels of access to opportunity.The medium score of staff nurses’ levels of structural empowerment increased with the increase of nurse managers’ levels of organizational citizenship behavior medium score. In the light of the present study results, it can be concluded that the
nurse managers’ levels of organizational citizenship behavior were high; the majority of staff nurses in the studied sample had highly levels of structural empowerment. Finally, there was a significant positive correlation between nurse managers’ levels of organizational citizenship behavior and staff nurses’ levels of structural empowerment at Menoufia University hospital at Shebin El-Kom, so when nurse managers display organizational citizenship behaviors this will result in empowering of their staff nurses. So, it is recommended that At the practice level:❖Nurse managers should provide staff nurses with clear essential information regarding to organizational goals, rules and best functional practices through distributing guiding brochures and posters at all organizational departments.❖Nurse managers should maintain availability of resources required to help staff nurses performing their job effectively such as equipment and supplies necessary for providing quality of patient care. ❖Encourage nurse managers and staff nurses to practice and apply organizational citizenship behavior as integral part of their daily work life through continuous workshops in the hospital.❖Hospital administration, in collaboration with staff development department should provide continuing education programs to update and reinforce nurse managers’ level about organizational citizenshipbehavior antecedents, importance, dimensions and outcomes and reinforce staff nurses’ information and skills about structural empowerment and finally achieve the main organizational goals.❖Nurse managers should be role model and encourage the health care team members (doctors-staff nurses-paramedical) to work together toward organizational goals rather than personal goals.❖Encourage nurse managers and staff nurses’ effective open communication links, participation in committees and ongoing meetings.❖Improve effective formal power domain through clear defined job description, reward and punishment and compensation system.❖Improve effective informal networks between nurse managers, staff
nurses, sponsors, peers and subordinates both within and outside the
organization.
At the educational level:❖Should involve organizational citizenship behavior concept in the curriculum of different years of undergraduate at faculty of nursing to improve and assert it among students through studying and clear understanding of its importance, dimensions and outcomes to nursing staff, patients and the overall organization.At the research level:❖Further research is needed to assess staff nurses’ level of organizational citizenship behavior.❖Further research is needed to assess relation between nurse managers’ level of organizational citizenship behavior and organizational productivity.
