Abstract

H. pylori is one of the most common gastrointestinal bacterial infections among humans. Infection is usually acquired during the first years of life through the fecal-oral and oral-oral route. H. pylori infections are related to some of the most common chronic clinical disorders of the upper gastrointestinal tract in humans. H. pylori was recognized as a major cause of chronic-active gastritis and is associated with duodenal and gastric ulcer disease .
Our aim in the present study was to evaluate the association between the presence of three polymorphisms; two of them in the non-coding promotor regions of IL-1 β host gene polymorphisms (at positions -511 and -31) and one in the ILRN gene and their association with the severity of gastritis in the presence of H.pylori infection. To achieve this goal, the present study was carried out on 100 symptomatic children who were referred for endoscopy because of hematemsis, abdominal pain and/or vomiting ; 50 of those children were diagnosed to have H. pylori infection by stool antigen and by histopathological findings of the obtained gastric biopsies and 50 children were diagnosed as H. pylori negative were included as controls.
Statistical analysis of data obtained from the present study showed the following results
1. Abdominal pain was the most common clinical manifestation in patients who were referred to our pediatric gastroenterology clinic for upper gastrointestinal endoscopy (86%) with statistical significance between H. pylori and abdominal pain.
2. Fasting epigastric abdominal pain with short duration (less than two monthes) was the most common type in H.pylori positive patients.
3. Family history of H.pylori gastritis was documented in (48%) of H.pylori positive patients especially the siblings followed by the mothers .
4. Microcytic hypochromic anemia was higher in H.pylori positive cases compared to H.pylori negative cases
5. Gastric erythema, antral nodularity, erosions and punctate hemorrhage were the common endoscopic findings among H. pylori patients.
6. The moderate chronic gastritis and and the mild active gastritis are the most common histopathological findings in H.pylori positive patients.
7. The C/T genotype of IL-1β-511 was predominant among H.pylori positive patients (72%) .
8. Persons who carry the gentype C/T of IL-1β-511 are 6.6 times more susceptible to be infected with H.pylori than the other genotypes.
9. H.pylori positive patients who carried the C/Tgenotype of IL-1β-511 are 8.3 times more susceptible to develope moderate to severe gastritis than the other genotypes.
10. Persons who carry 2 mutations are approximately 4 times more susceptible to be infected with H.pylori than persons who carry only one mutation
