Abstract
Summary and conclusion The aim of our study is to investigate the effect of sleeve gastrectomy on obese type 2 diabetic patients, its effect in remission of diabetes and possible role of gastrointestinal GLP1 and PYY hormones. This study was conducted on 30 obese T2DM patients with BMI ≥ 40 kg/m2. All patients had sleeve gastrectomy, surgeon used the same technique for all patients. Sixteen of our patients had partial remission of T2DM according to ADA remission criteria. Clinical evaluation in the form of: thorough history taking and medical examination including, blood pressure, pulse, temperature, head and neck, upper and lower limbs, chest, heart, abdomen, nervous system, nutritional and psychological examination, anthropometric measurements like weight, height, waist circumference, BMI calculation. We evaluated for routine laboratory investigation, HbA1c, thyroid function tests, ten-year ASCVD risk score calculation, helicobacter pylori serology (Antibodies), PYY, GLP 1 base line and 0.5 hour after MMT. No one had GERD on upper GI endoscopy. Follow-up visits were scheduled at 3, 6, and 9 months postoperatively with evaluation of % EWL, BMI and presence of late complications, HbA1C and lipid profile. Plasma PYY, GLP 1 basal levels and peak post-prandial response were measured at 15 days after surgery and 9 months postoperatively. Also we measured HbA1C by standardized techniques national glycohemoglobin standardization program and adopting DCCT trial assay method, HBA1C was done at 12 and 18 months to confirm for partial remission of diabetes.
