Abstract

Summary
Forty patients with different complex facial defects were included in this thesis; the patients were divided into five groups according to the sites of the defects in relations to the aesthetic units of the face. these groups are namely : cheek ( 12 cases) forehead ( 2 cases ) , nasal ( 10 cases ) , per orbital ( 10 cases ) per oral ( 6 cases )and forehead (2 cases) regions. The causes of facial defects were : (22 cases ) post-excision of neoplasm , (9 cases ) post traumatic, ( 3 cases ) post-burn and (6 case ) post excision of congenital lesion.
Different techniques were used for reconstruction of these defects including: local and regional flaps, tissue expansion, split- thickness and full thickness grafts.
The aesthetic and functional results were evaluated by patient satisfaction and physical examination. They rated excellent, good, satisfactory and bad in relation to the primary lesion. The functional results were good in 60%, satisfactory in 25% and bad in 15%.
The results were discussed and it was concluded that local and regional flaps are the ideal methods for reconstruction of facial defects because they give an excellent color and texture match. However, they can only be used in reconstruction of moderate sized defects. Tissue expansion can provide the surgeon with flaps to cover large defects. However, it can be used in benign lesions
Split or full thickness skin grafts proved to be an easy, simply and fast technique of reconstruction. However the aesthetic results are usually unsatisfactory.
