Abstract

Placental adhesive disorder is a catastrophic and life-threatening obstetric condition which is an important cause of intrapartum and postpartum hemorrhage and peripartum hysterectomy. Signs of diagnosis if placenta accreta are thinning of the myometrium underneath the placental loss of the retroplacental “clear space,” and interruption of baldder line by ultrasound and placental lacunae, tornado sign, increased periuterine vascularity between uterus and urinary bladder and dilated vessels around the cervix by Doppler ultrasound. Aim of the Work: This study was done to optimize ultrasound and doppler signs for diagnosis of placenta accreta. Patients and Methods This prospective study was conducted on pregnant women attending the outpatient clinic and inpatient Department of Obstetrics and Gynecology , Tanta university hospital. The study was carried out from December 2017 to December 2018. The inclusion criteria of this study were: Gestational age of pregnancy 20-24 weeks and all patients having anterior low lying placenta in primigravida or having anterior placenta previa with history of previous Cesarean section. For All selected patients , they were followed from 20 wks with 4 weeks interval till 36-38 weeks of gestation to detect gray-scale sonographic diagnostic criteria, and Doppler study findings. Cesarean section was done at 36-38 weeks gestations according to maternal and fetal condition , intraoperative observation of degree of placental adhesion, intraoperative complications was observed and those findings was correlated with sonographic findings. Results It can be noticed in the gray scale signs, ” loss of retroplacental clear space” sign was found in 28 patients at 28 weeks of gestation and in all patients (100%) at 32 and 36 wks of gestation and also in Doppler signs, ”placental lacunae” sign was found in 28 cases at 32 wks and 30 cases (100.00%) at 36 wks of gestation. However ”dilated vessels around the cervix” sign , only 2 cases was found at 36 wks of gestation
