Abstract

Infections due to C. trachomatis are the most common bacterial sexually transmitted disease, where a high prevalence of infection has been documented among otherwise healthy women. Endometritis as an intermediate state of such ascending chlamydial infection had been little studied and only a few cases of C. trachomatis associated endometritis had been reported . Since more than 50 % of C. trachomatis infections are asymptomatic, they may remain undetected and thus untreated for extended periods of time.
This study included one hundred consecutive patients complaining of infertility and /or dysfunctional uterine bleeding and showed picture of chronic endometritis at hysteroscopy and two endometrial biopsies were taken by hystroscope,75 specimens were confirmed endometritis by histopathology and the other 25 specimens were negative for endometritis. The 75 specimens were sent for detection of Chlamydial trachomatis by PCR,10 specimens were positive for Chlamydia and the other 65 specimens were negative for Chlamydia.
In this current study the prevalence of Chlamydia infection in patients with chronic endometritis was 13.3 %.
The mean age of the studied cases was 31.17±5.64 years, 6 patients (21.4%) in the studied group below 30 years were positive for C. endometritis and 4 patients (15.4%) above 35 years were positive for C. endometritis and no patients(o%) in between 30-35 years had C. endometritis.
No statistical significant correlation was found between positive cases of C. endometritis regarding to age .
There was statistical significant correlation between positive cases of C. endometritis and parity. The women who delivered 3 or more children were 24 (32%),those who proved positive for C. endometritis were 8 (33.3%) and the other16 ( 66.6%) were negative for C. endometritis.
There was statistically significant correlation between irregular uterine bleeding and presence of Chlamydia in patients with chronic endometritis. The women who had irregular uterine bleeding were 34(45.3%), those who proved positive for C. endometritis were 8 (23.5%) and the other 26 ( 76.5% ) were negative for C. endometritis. In the current study, the prevalence of C. endometritis in cases of infertility was 10 (13.2%) (primary infertility 2 (6.5%)-secondary infertility 1 (6.7%)).There was no statistically significant correlation between infertility and presence of Chlamydia in patients with chronic endometritis.
There was no statistical correlation between positive cases of C. endometritis regarding to positive or negative history of abortion.
