Abstract

Management of acute pancreatitis has been a challenge for surgeons , this study was conducted to evaluate the safety and efficacy of early laparoscopy in management of acute pancreatitis.114 patients with acute pancreatitis were presented to Emergency Surgery Unit between 1 March 2016 to 1 March 2018, of which 27 patients were diagnosed as severe pancreatitis and excluded from study, 6 patients were not fit anesthetically and 5 cases refused to participate. Laparoscopic management was conducted to 76 cases where laparoscopic drainage of abdominal exudate, opening of lesser sac and drainage were done to all cases. In case of biliary pancreatitis (54) plus drainage, LC was done. Following laparoscopic exploration there was a great statistical significance as regarding improvement of pain, leukocytic count, liver enzymes and pancreatic enzymes with significant p value.There were no intra-operative complications, post-operative mortality happened in two cases due to respiratory complications and ARDS. Conclusion:Early laparoscopy in mild to moderate acute pancreatitis is safe and efficient as regarding removing cause (GB), improving clinical picture, shortening hospital stay due to drainage of intra-abdominal inflammatory exudate, eliminating recurrent attacks and preventing re-admission for LC.
